Fermented Foods and Mental Health: A Systematic Review of the Evidence.
Review Question
1. What is the evidence supporting the use of fermented, non-alcoholic foods and beverages to decrease symptoms of depression, anxiety, or traumatic-stress among adults with depression-, anxiety-, and/or traumatic stress-related disorders? 2. What is the overall quality of trials conducted to date, particularly regarding study design, methodological quality, risk of bias, etc.? 3. Based on current evidence, what are knowledge gaps that should be addressed in future research?
Study Type Included
Peer-reviewed original research consisting of either randomized controlled trials (RCTs) or single-arm trials assessing symptom and/or diagnostic outcomes of fermented foods in adults with depression, anxiety, and/or traumatic stress-related disorders. Dissertations, conference proceedings, commentaries, editorials, letters, books, book chapters, and reviews will be excluded.
Condition Being Studied
Depression, anxiety, and traumatic stress-related disorders. A clinical diagnosis of one or more of the following disorders. Diagnosis must be verified by ICD-9/10 code(s), medical chart/health record, or via structured clinical interview: • Depressive Disorders: [Major Depressive Disorder (MDD), Clinical Depression, Persistent Depressive Disorder (PDD), Postpartum Depression (PPD)] • Anxiety Disorders: [Generalized Anxiety Disorder (GAD), Panic Disorder] • Trauma/Stress Disorders: [Post-Traumatic Stress Disorder (PTSD), Acute Stress Disorder (ASD)]

Intervention
TRIALS involving the consumption of fermented foods and/or non-alcoholic beverages. INCLUDES: - Fermented foods as they have historically been consumed in a human diet (e.g., kefir, kombucha) - Fermented foods that have been altered/processed to increase bacterial concentrations/activity (e.g., probiotic yogurt, raw/unpasteurized sauerkraut). - Vinegar (fermented from any substrate) when consumed as, or in a beverage. EXCLUDES: - Isolated acetic acid (the principal acid found in vinegar), also known as ethanoic acid, ethylic acid, vinegar acid, or methane carboxylic acid - Alcoholic beverages, including commercially available wine, beer, and spirits - Non-commercial alcoholic beverages, such as home-brewed beverages, and traditional Indigenous beverages (e.g., chicha, mahua, palm wine) - Supplements (e.g., probiotic supplements) - Isolated ingredients, substrates, or flavorings (e.g., soy protein extract, autolyzed yeast).

No comparison group required for inclusion.
Main Outcome
Restrictions are not based on timing or setting. Only studies including original data and published in a peer-reviewed journal are included. Changes in mental health metrics related to symptom TYPE, FREQUENCY, and/or SEVERITY (one or more of the following): • Self-administered, validated measures of symptoms related to depression, anxiety, and/or trauma-related stress disorders. • Clinician-administered (inpatient or outpatient) assessments of symptoms related to depression, anxiety, and/or trauma-related stress disorders. Measures of effect No specific measures comprise the focus - see above.
