Efficacy and safety of Bufei Huoxue Capsule in the treatment of pneumoconiosis: a systematic review and meta-analysis.
Review Question
Pneumoconiosis is accepted as one of the most lethal occupational diseases. For decades, the World Health Organization (WHO) and International Work Organization (ILO) have been focused on eliminating pneumoconiosis globally. To date, pneumoconiosis is incurable. Novel approaches to effective treatment are urgently needed. In recent years, natural drugs or Chinese herb extracts in pneumoconiosis treatment has been steadily acknowledged. It is reported that Traditional Chinese Medicine (TCM) has a good therapeutic effect on pneumoconiosis, which can improve quality of life, respiratory function, and oxidative stress. Bufei Huoxue Capsule is composed of Astragalus membranaceus, red peony, and Psoralea corylifolia. It is clinically suitable for the supplementary treatment of pneumoconiosis. This study was aimed to systematically assemble the evidence of pneumoconiosis treatment with Bufei Huoxue Capsules to provide reference for clinicians
Study Type Included
All published RCTs for patients with pneumoconiosis by BFHXC were included. The exclusion criteria were: (1) Non-RCTs including conference reports, review literature, animal tests, and theoretical experience; (2) incomplete data and duplicated data; (3) experimental group treated with other Chinese medicines.
Condition Being Studied
Pneumoconiosis is a collection of various occupational lung diseases caused by inhalation of airborne dust and fibers. Exposures to asbestos, silica, and coal dust are well acknowledged risk factors for pneumoconiosis. Among the current occupational diseases in China, there are 12 kinds of statutory pneumoconiosis, which can all eventually induce irreversible lung damage and has the potential to cause progressive physical disabilities. At present, large number of industrial applications and working activities involving the use or handling of silica containing materials, an estimated number of millions of workers are exposed to harmful dust worldwide.
Participant
All published RCTs for patients with pneumoconiosis by BFHXC were included, without limitations of gender, race, and age. The diagnosis of pneumoconiosis should conform to the national diagnostic standard of pneumoconiosis or its previous version (GBZ 70-2002/GBZ 70-2009), regardless of the specific classification and stage of illness.

Intervention
The treatment group of pneumoconiosis patients was treated with Bufei Huoxue Capsule.
Comparator Control
Patients use conventional treatment, which includes oxygen therapy, Western Medicine such as ICS, LAMA, LABA, and expectorant drug, antispasmodic drugs and placebo granule.
Main Outcome
The outcome measures included functional capacity, and dyspnea score, such as modified Medical Research Council (mMRC) dyspnea scale and chronic obstructive pulmonary disease (COPD) Assessment Test(CAT).

1. Partial pressure of oxygen and carbon dioxide in arterial blood. 2. Immunity-related Indexes in blood. 3. Quality of life evaluation, such as St. George's Respiratory Questionnaire (SGRQ). 4. 6-minute walking distance (6MWD). 5. Clinical efficacy, etc.
