Review Question
Is Chinese herbal medicine Dengzhan Xixin injection effective and safe for treatment of acute ischemic stroke?
Study Type Included
We included only randomized controlled trials (RCTs) studying Dengzhanxixin injection for acute ischemic stroke. Quasi-RCTs which allocated the patients by dates of birth, or day of the week, alternation, the sequence of admission, case record numbers were excluded. Both published and unpublished trials were included. We applied no restrictions regarding the language of publication and combined multiple publications reporting the same trial into one study.
Condition Being Studied
Stroke is a neurological impairment due to an acute focal injury of the central nervous system (CNS) by a vascular cause, including cerebral infarction, intracerebral hemorrhage (ICH), and subarachnoid hemorrhage (SAH). It often leads to physical disability and death. According to the World Health Organization, stroke has become the second most common cause of death across the world. Acute ischemic stroke or cerebral infarction is the most common type of stroke, accounting for 60 to 80 percent of all strokes, so we focused on acute ischemic stroke in this research.
Participant
Participants who were diagnosed with acute ischemic stroke by computerized tomography (CT) or magnetic resonance imaging (MRI) scan regardless of their sex, age or race were eligible.

Intervention
Chinese herbal medicine Dengzhan Xixin injection (extraction of Erigeron breviscapus) was regarded as the intervention, regardless of its dosage, duration.
Comparator Control
Placebo, or a Western therapy. We also allowed a co-intervention (conventional western medicine) if applied in all arms.
Main Outcome
(1) Death (2) Disability or dependence defined as Barthel Index (BI) scores <60 or the modified Rankin scale (MRS) scores >3 (People who are severly disabled and have to depend on help from other people for their activities of daily living). (3) Serious adverse events: fatal, life threatening, requiring change of treatment regimen. Measures of effect at the end of the scheduled treatment or follow up period

(4) Changes of neurological deficit after treatment. The measures can focus on specific impairment (e.g., motor deficit or cognitive impairment) or global neurological deficit (e.g., the National Institutes of Health Stroke Scale (NIHSS); European Stroke Scale (ESS); the Neurological Function Deficit Score (NFDS), a nationwide accepted scoring system recommended at the Fourth National Cerebrovascular Diseases Conference in China[15]; or the Scandinavian Stroke Scale (SSS)). (5) Quality of life if assessed in the included trials by validated scale without limitations on the specific scales used. (6) Treatment failure, defined as reduction of NFDS less than 18% or death according to the nationally approved criteria in China. (7) Non-serious adverse event specifically associated with Dengzhan Xixin injection such as allergic reaction, palpitation, flushing, etc. Measures of effect At the end of the scheduled treatment or follow up period
