Effects of pomegranate on the cardiorespiratory system in healthy individualsundergoing aerobic exercise: a systematic review with meta-analysis.
Review Question
1 – Does supplementation with pomegranate by exercise practitioners have a statistically significant effect on cardiorespiratory and/or cardiometabolic responses and parameters? 2 – Does supplementation with pomegranate or its bioactive compounds by exercise practitioners have a statistically significant effect on responses and biochemical parameters, biomolecular markers, inflammatory markers and/or signalizers? 3 – If yes, what is the period of exposure and dose required to achieve the results? 4 – Are the results, if positive, altered according to the age group of the population studied? 5 – Considering the ways in which the pomegranate intervention is provided for administration or consumption (juice, capsule and others) which one can provide the best results for the mentioned outcomes? 6 – Considering the ways in which the pomegranate intervention is provided for administration or consumption (juice, capsule and others) what would be the concentration needed to attain the possible statistically significative results? 7 – Does the intensity and exposure period of the exercise tests performed in each study imply differences in results?
Study Type Included
Randomized clinical trials that has at least one comparative group that did not receive the intervention with pomegranate and the same exercise protocol, in volume and intensity as the treated group.
Condition Being Studied
Cardiorespiratory aptitude is considered one of the primordial factors for promotion and maintenance of health, thus establishing association to the occurrence of some comorbidities, such as elevation of systemic arterial pressure, heart rate variability and reduction of oxygen uptake, transport and utilization capacity in the body (VO2max). Such parameters, when compromised, may affect the quality of life, well-being, organic functionality of individuals and tasks of daily life as activities of daily life and work. Moreover, keeping in balance or stimulating the continuous improvement of parameters related to cardiorespiratory fitness still helps to prevent sedentariness and obesity. The increase of lactate levels in the bloodstream interferes with the continuity of exercise practice, the last being one of the main factors to promote and maintain the health of individuals. Thus, these parameters are closely linked to cardiovascular/cardiometabolic health.
Participant
Inclusion: healthy individuals active in physical exercise and/or any sport modality before their participation in the study (this way we can infer that the participants were not sedentary, eliminating possibilities of recruiting people with possible pathology potential and biasing the sample) being able to understand different levels of exercise, such as recreationists, amateurs/intermediates and professional/elite/advanced athletes. Will be considered individuals aged 18 and over. Exclusion: popultation that presents pathology and, when healthy, but sedentary, under 18 years old, with a history of pathologies that directly impact the cardiorespiratory/cardiometabolic system, such as COPD, asthma, tuberculosis, pneumonia and other diseases related to this system.

Intervention
The Punica granatum Linnaeus. species has a great therapeutical potential. It is necessary to consider that the fruit is divided in parts, specially when it is related to clinical researches. Therefore, considering each part of the fruit, they develop different functions among themselves, much likely because of the bioactive compounds found within them. In the present research, we will consider only studies that used the pulp, region that involves the seeds also known as albedo or sarcotesta. In the recent literature is described that this portion of the fruit is rich in polyphenols, flavonoids, terpenoids, ellagitannins, punic acid and punicalagin. Intake of pomegranate pulp juice, or capsule supplementation thereof, as well as other forms which arise during official screening shall be considered as intervention.
Comparator Control
Placebo groups will be considered as a comparison group. These groups must also have undergone the physical exercise intervention.
Main Outcome
The information described here are based on our preliminary searches, therefore, we clarify that the final outcomes will only be defined after the entire screening stage has been carried out, given the small number of studies rescued. The results will depend on the analysis of methodological homogeneity among other factors, but it is predicted according to preliminary searches that may include VO2 max, systemic arterial pressure, heart rate and lactate. The VO2 max measures the functional capacity of the cardiorespiratory system, besides being the main predictor of cardiovascular diseases, it is measured by means of treadmill or cycloergometer stress tests. Systemic blood pressure and heart rate are linked to this parameter. The intervention with ingestion of pomegranate together with physical exercise will be of fundamental importance to identify possible drops in blood pressure levels and heart rate variability. Lactate levels increase significantly according to the intensity of the exercise, and an inversion of production versus removal may occur, thus implying continuity of exercise. This parameter is verified by analysis of blood samples from the participants. Commonly, all parameters are measured and collected before, and/or during and/or after the intervention. Measures of effect There is an association between the consumption of Punica granatum L. and the improvement of cardiorespiratory and/or cardiometabolic outcomes. However, the outcomes can only be reported after the official screening of the articles.
