Review Question
What is the effectiveness of leech preparations on nephrotic syndrome patients with hypercoagulable and hyperlipidemia?
Study Type Included
Inclusion Criteria: The study is a randomized controlled trial (RCT). Exclusion Criteria: Non-RCT trials.
Condition Being Studied
Nephrotic syndrome is a clinical syndrome of hyperlipidemia, edema, and various complications due to increased glomerular filtration membrane permeability leading to massive proteinuria and hypoalbuminemia. Among them, hypercoagulability and hyperlipidemia are typical characteristics of NS. Due to platelet activation, enhanced erythrocyte aggregation, and imbalance between procoagulant and anticoagulant factors, blood hypercoagulability is prone to occur. Hypoproteinemia promotes increased lipoprotein synthesis in the liver, leading to hyperlipidemia. Hyperlipidemia can make the blood thicker and promote thrombosis. Leech contains hirudin, a natural thrombin-specific inhibitor that can act as an anticoagulant, lower blood lipids, anti-inflammatory, anti-fibrosis, improve renal function, and protect the kidneys.
Participant
We will include systematic reviews of randomised controlled trials on human subjects diagnosed with Nephrotic syndrome.

Intervention
Inclusion Criteria: Leech preparations. (leech powder, leech injection, pulse blood sampling, and other symptomatic treatments) Exclusion Criteria: Use other Chinese medicine treatments besides leech preparations.
Comparator Control
Inclusion Criteria: Routine treatment (glucocorticoids, immunosuppressive agents, anticoagulation, and other symptomatic treatments) Exclusion Criteria: Use other Chinese medicine treatments.
Main Outcome
We will be evaluating improvement or decline from baseline in quantitative and objective measures. Main outcome: Reducing 24h urine total protein, total cholesterol, triglyceride, fibrinogen, increasing plasma albumin. Measures of effect Mean Difference
