- We will include randomized trials to assess the effects of Silybum marianum extract on bodyweight, and will supplement these with case– control studies for the assessment of harms.

- Inclusion: Adults Exclusion: Adolescents (under 18 years of age), elderly (over 70) and Adults with acute or chronic fatal conditions.

- whole Silybum marianum extract- silymarin- silibinin supplement daily for 4 weeks to 12 weeks.
- Comparator Control
- Placebo, adults not receiving and weight changing medication or supplements.
- Main Outcome
- Change in bodyweight from baseline to the last available follow-up, measured in kg. Measures of effect weighted mean difference

- Additional Outcome
- If possible, body weight change in overweight/ obese subgroup, metabolic syndrome patients’ subgroup, NAFLD patients’ subgroup. Measures of effect weighted mean difference
