Chinese herbal medicine Lu Gen (Phragmites communis) for acute respiratory tract infections: A systematic review
Review Question
To determine the effectiveness and safety of Chinese herbal medicine Lu Gen (Phragmites communis) or herbal formulations containing Lu Gen in the treatment of acute respiratory tract infections (ARTIs).
Study Type Included
Randomised controlled trials (RCTs) will be included. Cross-over trials will be included if comparative data prior crossing over are available. If the trial evidence is sparse, we will consider including case-control studies and cohort studies.
Condition Being Studied
Acute respiratory tract infections (ARTIs) are one of the leading cause of morbidity and mortality worldwide, with most people experiencing at least one episode of ARTI each year. ARTIs are composed of acute upper respiratory tract infections(AURTIs)and acute lower respiratory tract infections (ALRTIs). The current treatment methods mainly include symptomatic therapy, antiviral drug therapy, anti-bacterial drug therapy, and glucocorticoid therapy. However antibiotics are of limited benefit in majority of uncomplicated infections (Pichichero ME, 2000) and there is inadequate evidence supporting the effectiveness of its use in AURTIs (Spurling GKP, 2017). Traditional Chinese medicine has shown benefits in the treatment of ARTIs. The study is to explore the alternative or complementary therapy which can reduce the use of antibiotics while relieve the symptoms of ARTIs.
Participant
Patients in all age groups, diagnosed with ARTIs or presented with ARTIs symptoms, will be included. A clinical diagnosis of ARTIs is the main inclusion criteria. According to the International Statistical Classification of diseases (ICD-10), ARTIs include the acute nasopharyngitis, acute sinusitis, acute pharyngitis, acute tonsillitis, acute laryngitis and tracheitis, acute upper respiratory tract infection at multiple sites and not specified sites, acute bronchitis, common cold, influenza, pneumonia. We will exclude pneumonia with the exception of acute exacerbation of chronic obstructive pulmonary disease (AECOPD). The most common symptoms of ARTIs include headache, nasal congestion, nasal discharge, dry throat, fever, cough, sore throat, itchy throat, sneezing, fatigue, headache and chills, muscle ache.

Intervention
P. communis (Lu Gen in Chinese), usually the root as the therapeutic part, has a nature of clearing heat and fire, nourishing yin and producing body fluid in Traditional Chinese Medicine (TCM). The most commonly used form of it is herbal mixture, not usually as a single herb. Any preparation of the root from P. communis, either as a single herb, herbal extracts or within a formula which includes oral, nasal, or external use, apart from injection will be included. There will be no restrictions concerning dosage, methods of dosing or duration of treatment. Exclude the combination of non-drug therapy such as massage, acupuncture. A study which do not provide the compose of the herbal formula or the condition last more than 4 weeks will be excluded.
Comparator Control
No treatment, placebo, usual treatment such as antipyretics, antivirals, or anti antibiotics. Trials evaluating P. communis in combination with other Chinese medicine treatment(s) will be excluded.
Main Outcome
The time to disappearance of the main symptoms, or the proportion of patients with main symptom resolved within 5 days. The main symptoms of interest are headache, cough,fever, nasal congestion, nasal discharge, dry throat, sore throat, dry throat, itchy throat, and sore muscles. Measures of effect Mean differences (MD) or standardized mean differences (SMD) and 95% confidence intervals (CI), Risk ratio (RR) and 95% CI.

1、cure rate 2、Time spent absent from school or work due to illness 3、adverse events An emergent approach to additional outcomes will be taken. Studies that do not report either our main or additional outcomes will also be included. Measures of effect These will be the same as for the main outcomes.
