- Review Question
- To evaluate the effectiveness and safety of moxibustion treatment for alleviating pain and improving lower limb function for patients with knee osteoarthritis
- Study Type Included
- We will include randomised controlled trials (RCTs). For randomised cross-over trials, only phase 1 data will be included in the review because treatment carry over effects are likely.
- Condition Being Studied
- Knee osteoarthritis is a major cause of pain and functional limitation. Complementary and alternative medical approaches have been employed to relieve symptoms and to avoid the side effects of conventional medication. Moxibustion has been widely used to treat patients with knee osteoarthritis.
- Participant
- We will select studies that included participants aged 18 years or over with a diagnosis of knee osteoarthritis according to the criteria from the American College of Rheumatology and knee pain for at least three months.We will exclude studies that include participants who have undergone total knee replacement.

- Intervention
- Moxibustion is a traditional Chinese method of acupuncture treatment, which utilizes the heat generated by burning herbal preparations containing the plant Artemisia vulgaris (it is also called Mugwort or Moxa; the latter is derived from the Japanese moe kusa which means burning herb) to stimulate the acupuncture points. The technique consists of lighting a moxa stick and bringing it close to the skin until it produces hyperaemia due to local vasodilatation. Studies that involved any form of moxibustion (e.g. direct moxibustion, indirect moxibustion, heat-sensitive moxibustion, moxa burner moxibustion, warm needling or natural moxibustion, etc.) as the sole treatment or as a major part of a combination therapy with other intervertion (e.g. conventional drugs, etc.) will be included. And the studies in which moxibustion is used as an adjuvant therapy will be excluded.
- Comparator Control
- The intervention in the control group may include a placebo, no treatment or other conventional treatments that are currently accepted and widely used for patients.Other herbal medicines or complementary medicines will not be accepted as a control intervention.
- Main Outcome
- The most important outcome will be pain intensity and function (muscle strength, mobility, stability). Measures of effect There are multiple outcome measures that have been established in Knee OA including the Oxford Knee Score, the Lysholm Knee Scores, the International Knee Documentation Committee (IKDC) Scores, the Hospital for Special Surgery (HSS) Score, and the Western Ontario and McMaster universities (WOMAC) OA Index. Also standardized measures of pain (e.g. the Visual Analogue Scale) will be included.

- The secondary outcomes will be the proportion of participants whose symptoms improved after treatment, knees deformity, adverse events during or after the study, etc. Other outcomes according to the authors' reports.
