Review Question
What is the diagnostic accuracy of lactate dehydrogenase/adenosine deaminase ratio in differentiating tuberculous pleural effusion and parapneumonic effusion?
Study Type Included
Studies of patients having both tuberculous and parapneumonic pleural effusions and reporting on diagnostic performance of pleural fluid lactate dehydrogenase/adenosine deaminase ratio. Observational studies (cross-sectional, cohort and case–control studies) will be included.
Condition Being Studied
Microbiological confirmation of tuberculous pleural effusion is often challenging and diagnosis often relies on pleural fluid biochemical characteristics or histopathological findings via invasive biopsy methods. Pleural fluid adenosine deaminase has been shown to correlate with a diagnosis of tuberculosis, but as a standalone test may be raised in other pleural infection. Parapneumonic effusion may present in a similar way to tuberculous effusion. More recently, pleural lactate dehydrogenase to adenosine deaminase ratio was explored as a discriminator between tuberculous and parapneumonic effusion. However, there is no clear data on the diagnostic accuracy of the lactate dehydrogenase/adenosine deaminase ratio for this purpose.

Summary diagnostic accuracy (sensitivity, specificity, diagnostic odds ratio) of pleural fluid lactate dehydrogenase/adenosine deaminase ratio in differentiating tuberculous pleural effusion from parapneumonic effusion. Measures of effect Summary sensitivity, summary specificity and summary diagnostic odds ratio for the individual tests, and the summary relative diagnostic odds ratio (RDOR) for the comparative analysis
