- What is the efficacy and safety of trichloroacetic acid treatment in adults with xanthelasma palpebrarum?
- Study Type Included
- We will include randomized trials and observational studies (including cohort and case–control studies) to assess the beneficial effects of the treatments and assessment of harms.
- Condition Being Studied
- Xanthelasma palpebrarum is the most common cutaneous xanthoma. They most often occur in middle-aged and older adults and are associated with dyslipidemia in approximately 50% of the patients. Clinically, it presents as yellowish soft plaques or nodules that usually appear symmetrically on the medial side of the upper eyelids but can also involve the lower eyelids.
- Participant
- Inclusion: Adults with clinical diagnosis of xanthelasma palpebrarum. Exclusion: Under 18 years old, pregnancy and medical history of keloid or hypertrophic scar formation.

- Trichloroacetic acid treatment intervention is defined as the direct application of chlorinated acetic acid at different concentrations (30%-100% in one or more sessions) to xanthelasma palpebrarum lesions used as tissue cauterant that works by dissolving lipids and coagulating proteins.
- Comparator Control
- Usual care (Laser, cryotherapy, radiofrecuency or surgery)Laser. Destruction of perivascular foam cells via thermal energy damage and coagulation of dermal vessels leading to blockage of further lipid leakage into tissue. Cryotherapy. Ice crystal formation, thermal shock, enzyme inhibition, cellular dehydration and electrolyte disruption Radiofrequency ablation. Works by causing vaporization at a cellular level in tissues. It initiates fibrosis followed by volume reduction during healing. Surgery. Percutaneous excision of the lesions followed by direct suture.
- Main Outcome
- Efficacy defined as any clinical improvement after trichloroacetic acid application. Complete response: disappearance of the lesion. Partial response: cosmetic improvement of the lesion using a grading scale of clearance (>75%, 50–75%, 25–50% or <25% cleared). Safety. Benefits outweigh the possible risks for most people. Adverse effects: edema, pain, erythema, post-inflammatory hypo- or hyperpigmentation, crust formation and scarring. Measures of effect Clinical result ratio

- Recurrence of lesion at 6 months after treatment with trichloroacetic acid application, laser, cryotherapy, radiofrequency ablation or surgery. Measures of effect Clinical result ratio
