To evaluate the accuracy of smart phone image capture of the cervix after the application of acetic acid and / or Lugol’s iodine in the diagnosis of CIN 2 + in women undergoing cervical screening and assessment Population: Women with positive cervical screening (either HPV, cytology, VIA) who undergo cervical assessment, image capture, and diagnosis Setting: Community health clinics, hospital-based settings, all income level countries Index test: Smart phone cervical imaging after application of acetic acid and / or Lugol's iodine Reference standard: Cytology, histopathology Target condition: CIN 2 +
Study Type Included
The ideal study design for diagnostic test performance is cross-sectional where the test is performed on consecutively assessed patients is cross-classified with disease status and a reference test. Although less ideal, randomised control trials that use previously independently assessed tests will also be considered.

Smart phone cervical imaging after application of acetic acid and / or Lugol's iodine
Comparator Control
While the ideal reference standard for the diagnosis of CIN 2+ is histology, we will also consider colposcopy as in many low and middle income countries the treatment of cervical disease is based on visual inspection alone and is done with cryotherapy, without confirmatory histopathology.
Main Outcome
Primary objective: to evaluate the accuracy of smart phone image capture of the cervix after the application of acetic acid and / or Lugol's iodine in the diagnosis of CIN 2 + in women with positive cervical screening The outcome is defined as a positive screening test for CIN 2+ if the images captured by the smart phone are considered to be positive (demonstrative of CIN 2+) and comparison to the reference standards will be made. Measures of effect We will plot the correlation coefficient (or sensitivity) of smart phone image capture of the cervix along with the 95% confidence intervals (CIs) using forest plots.

- Secondary objectives 1. To compare the accuracy of smart phone images with traditional colposcopy 2. To describe the potential barriers to implementation of the technology, including quality of the images 3. To assess the patient acceptability of smart phone use in cervical screening Measures of effect 1.
