- There is a range of lesions, similar to oral lichen planus called lichenoid, that may be mistaken for a differential diagnosis.Lichenoid lesions are divided into four groups. 1) lichenoid contact lesions 2) drug related lichenoid lesions, 3) lichenoid lesions in chronic graft versus host disease (cGVHD), and 4) lesions that have a lichen planus like aspect, but that lack one or more characteristic clinical aspects. Lichen planus is a chronic systemic disease of established immune-mediated pathogenesis. It commonly involves the mucosa of the oral most often in the absence of skin lesions. it is more frequently seen in women.They generally appear with white lines and are usually accompanied by erythema, erosion, white plaques, or atrophy that occur bilaterally. Oral lichenoid lesions, on the other hand, may be unilateral and occur in the presence of various dental materials or following the administration of certain medications. The clinicopathologic features of lichen planus lesions and oral lichenoid lesions are similar and represent a form of hypersensitivity reaction that is mainly caused by dental restorations or drugs.There is a risk of oral cancer in patients with oral lichenoid, but this increased risk does not appear to be present in patients with OLP.
