- Review Question
- In people living with chronic obstructive pulmonary disease (COPD): 1) What is the prevalence of glucocorticoid-induced hyperglycaemia and diabetes? 2) What is the incidence of glucocorticoid-induced hyperglycaemia and diabetes? 3) What is the relative risk of glucocorticoid-induced hyperglycaemia and diabetes compared to those not exposed to systemic glucocorticoids? 4) What are the characteristics of the dose-response relationship between systemic glucocorticoid use and new-onset diabetes (people without diabetes) and hyperglycaemia (people with diabetes)? 5) Do the observed associations vary according to study design, diabetes status at baseline, treatment duration and year of publication, method for ascertaining diabetes and hyperglycaemia, body mass index (BMI) and maintenance treatment for COPD?
- Study Type Included
- Inclusion: Randomised controlled trials (RCTs), single arm trials, and observational analyses of RCTs; in trials, glucocorticoid-induced diabetes/hyperglycaemia will typically be a secondary or safety outcome rather than primary outcome, cohort studies (prospective and retrospective), case-control studies (report measures other than incidence), cross-sectional studies (prevalence). Observational studies will also include pharmacoepidemiological studies of real-world data. Exclusion: Study design does not belong to any of the following categories: Single arm trials, RCTs and observational analyses of RCTs; cohort studies (prospective and retrospective), case-control studies, cross-sectional studies.
- Condition Being Studied
- In people living with COPD we will study: 1) Glucocorticoid-induced diabetes defined as a new-onset diabetes following exposure to glucocorticoids in people without pre-existing diabetes; and 2) Glucocorticoid-induced hyperglycaemia defined as worsening hyperglycaemia in people with diabetes following exposure to glucocorticoids.

- Inclusion: Adults (aged 18 years or older) diagnosed with COPD, with or without documented diabetes mellitus at baseline documented (i.e. information on personal history of diabetes at study entry) Exclusion: Children (aged < 18 years)

- Inclusion: Systemic use of glucocorticoids (oral, intravenous or intramuscular administration) including dexamethasone, prednisone, prednisolone, methylprednisolone, hydrocortisone, budesonide, cortisone, deflazacort, plenadren and triamcinolone (15) equivalent to prednisolone 5 mg daily or higher for 3 or more days with or without a concomitant use of topical glucocorticoids. Exclusion: Topical use of glucocorticoids (inhaled, eye/ear drops, per rectum, intraarticular, dermal, etc.) without concomitant systemic glucocorticoids or systemic glucocorticoid treatment of less than 3 days

- • Diabetes mellitus (new onset in people without prior diagnosis of diabetes mellitus) as defined in each study • Hyperglycaemia (worsening glycaemic control in people with diabetes mellitus before starting glucocorticoids) as defined in each study Measures of effect Reported hazard ratios, relative risks, or odds ratios with confidence intervals
