- Title
- Is the use of topical gel containing 0, 2% Chlorhexidine, Chitosan, Dexpanthenol and Allantoin effective in controlling pain, edema, trismus and healing in patients undergoing removal of third molars? Systematic Review and Meta-analysis
- Review Question
- P- Patients submitted to third molar surgery; I- Use of topical gel containing 0, 2% Chlorhexidine, Chitosan, Dexpanthenol and Allantoin; C- Placebo, Without tratment or other control medicine; O- Pain, edema, trismus and healing.
- Study Type Included
- The following were the inclusion criteria: (1) randomized clinical trials; (2) studies with an intervention group that received use of topical gel containing 0, 2% Chlorhexidine, Chitosan, Dexpanthenol and Allantoin a control group that received a placebo, without tratment or other control medicine in area outside the operation site; (3) studies that evaluated postoperative pain, edema, trismus and healing; (4) studies published in English, Spanish or Portuguese; (5) no restriction regarding date or time of publication.
- Condition Being Studied
- The removal of third molars is among the most common surgical procedures performed by oral-maxillofacial surgeons. This procedure is associated with expected events of pain, swelling and trismus, which cause important structural and functional changes, as the surgical site is composed mainly of loose connective tissue with numerous blood and lymphatic vessels. The main analgesic agents indicated following the removal of third molars are acetaminophen, non-steroidal anti-inflammatory drugs or a combination of novel medications or therapies. Despite the benefits of these medications, many patients do not tolerate their use due to the side effects, such as gastric or duodenal ulcers, platelet disorders, kidney impairment and bronchospasms. Moreover, caution is required regarding the use of these drugs on pregnant women, nursing mothers, diabetics, immunosuppressed individuals and those with vascular diseases. Therefore, the adequate selection of an analgesic and its administration route are fundamental.
- Participant
- The following were the inclusion criteria: (1) randomized clinical trials; (2) studies with an intervention group that received use of topical gel containing 0, 2% Chlorhexidine, Chitosan, Dexpanthenol and Allantoin a control group that received a placebo, without tratment or other control medicine in area outside the operation site; (3) studies that evaluated postoperative pain, edema, trismus and healing; (4) studies published in English, Spanish or Portuguese; (5) no restriction regarding date or time of publication. Studies are excluded for the following reasons: (1) in vitro studies; (2) animal studies; (3) cohort, case-control, literature review, case report and case series studies; (4) other types of documents, such as letters to the editor, abstracts presented at conferences and book chapters; (5) studies that did not evaluate the primary or secondary outcomes; (6) studies that did not describe the route of 0, 2% Chlorhexidine, Chitosan, Dexpanthenol and Allantoin; (7) studies not related to the subject; (8) studies with complete texts not available in databases.

- Human Disease Modelled
- Intervention
- Use of topical gel containing 0, 2% Chlorhexidine, Chitosan, Dexpanthenol and Allantoin.
- Comparator Control
- Placebo, Without tratment or other control medicine.
- Main Outcome
- Pain, edema, trismus and healing. Measures of effect The results will be considered statistically when the P value was <0.05 and 95% confidence intervals (CI) were calculated. The heterogeneity between the studies will be determined by the χ² test (chi²) and the Higgins inconsistency test (I²). The results of the chi² test will be considered statistically significant when the P value was <0.10, as this value may indicate heterogeneity. The I² test with values above 50% indicates moderate heterogeneity and values above 75% indicate high heterogeneity.
- Outcome Measure
- Additional Outcome
- complications and number of rescue painkillers.
