Review Question
What is the comparative efficacy and safety of Dexamethasone and Magnesium Sulfate as an adjuvant to local anaesthetic in peripheral nerve blocks?
Study Type Included
Inclusion criteria: • All randomised control trials comparing efficacy and/or safety of Dexamethasone and Magnesium Sulfate as an adjuvant to local anaesthetic in any kind of peripheral nerve blocks. Exclusion criteria: • Studies administering Dexamethasone and Magnesium Sulfate through neuraxial and other routes (e.g, intravenous administration) • Studies using autonomic nerve blocks • Observational and non-interventional studies, case series, case reports, review articles • Single-arm studies • Duplicate studies (in such cases, studies with the most up-to-date and largest data will be included)
Condition Being Studied
Peripheral nerve blocks are used for providing anaesthesia in specific body area for conducting surgery and performed either by landmark or ultrasound guided technique. Local anaesthetic blocks transmission in nerve pathways. Adjuvants are the agents which are used to enhance the quality and efficacy of regional techniques.These work synergistically to quicken the onset, increase duration of analgesia, improve quality of analgesia, and limit potential medication related adverse effects. Dexamethasone and magnesium sulfate has also shown to prolong the duration of block and post-operative analgesia when added to local anaesthetic in various upper (interscalene, suprascapular, infraclavicular, axillary nerve block ) and lower extremity (lumbar plexus block, femoral nerve block, fascia iliaca, obturator nerve, popliteal, saphenous) regional blocks. Adjuvants like dexmedetomidine, dexamethasone, fentanyl, magnesium sulfate has also shown to enhance the quality and efficacy of local anaesthetic in abdominal nerve blocks (transversus abdominis plane, subcostal, rectus, ilioinguinal, transversalis fascia plane, and quadratus lumborum block). Studies have compared the efficacy and safety of Dexamethasone and magnesium sulfate as an adjuvants in Peripheral nerve blocks for postoperative analgesia. So, we planned a meta-analysis to compare analgesic efficacy and safety of Dexamethasone and magnesium sulfate as adjuvants in in peripheral nerve block for postoperative analgesia .
Participant
Inclusion criteria: • The adult population of more than 18 years undergoing surgery of upper limb, lower limb, abdominal, gynaecological or thoracic surgery under regional anaesthesia or general anaesthesia. • Patients undergoing any peripheral nerve block (nerve plexus block of either brachial, lumbar or cervical, myofascial plane block, thoracic nerve blocks etc ) and any administration techniques (blinded or USG guided approach) Exclusion criteria: • Patients undergoing emergency Surgery • Patients on either chronic or acute dexamethasone and magnesium sulfate medication. • Paediatric population

• Dexamethasone as an adjuvant to local anaesthetic in nerve block regardless of dosage, volume or type of local anaesthetic
Main Outcome
Main Outcome(s) • Duration of analgesia: Meta-analytic summary of mean difference (95% CI) in the duration of analgesia between dexamethasone and magnesium sulfate as an adjuvant to local anaesthetic in peripheral nerve block. • Onset of sensory and motor block: Meta-analytic summary of mean difference (95% CI) in the onset of sensory and motor block between Dexamethasone and Magnesium sulfate as an adjuvant to local anaesthetic in peripheral nerve block. • Duration of sensory and motor block: Meta-analytic summary of mean difference (95% CI) in the duration of sensory and motor block between Dexamethasone and Magnesium sulfate as an adjuvant to local anaesthetic in peripheral nerve block
Outcome Measure
Additional Outcome
• 24 hr Post operative analgesic consumption • Adverse events: Total number of adverse events (Sedation, Intraoperative or postoperative complications, Pneumothorax, Vascular puncture, Horner syndrome, Somnolence, Bradycardia , Hypotension, Post operative nausea vomitiong) in each study arm will be considered as a numerator and the total number of patients in each study arm will be considered as a denominator. The meta-analytic summary will be presented as a risk ratio (95% CI). • Quality of Anaesthesia : Meta-analytic summary of risk ratio (95% CI) in the quality of anaesthesia between dexamethasone and magnesium sulfate as an adjuvant to local anaesthetic in peripheral nerve block • Patient satisfaction score : Meta-analytic summary of risk ratio (95% CI) in patient satisfaction score between dexamethasone and magnesium sulfate as an adjuvant to local anaesthetic in peripheral nerve block
