Efficacy and safety of lowmolecular-weight heparin in chinese patients with sepsis: systematic review and meta-analysis
Review Question
Clinical studies have shown that heparin in patients with sepsis may be associated with decreased mortality. The purpose of this study is to evaluate the efficacy and safety of lowmolecular-weight heparin in Chinese patients with sepsis. Meanwhile, the current research status was analyzed to provide information for future studies.

Inclusion Criteria: Prospective, randomized, controlled trial Exclusion Criteria: (1)Studies involving animals; (2)Quasi-randomized controlled trials (e.g. alternate randomization, randomization according to hospital number) and non-randomized trials (e.g. cohort and case-control studies)
Condition Being Studied
Sepsis is a life-threatening organ dysfunction caused by an imbalance in the body's response to infection. The pathogenesis of sepsis is complex, involving many issues such as infection, inflammation, immunity, coagulation, and tissue damage, and is closely related to the pathological and physiological changes of multiple systems and organs in the body. In recent years, research has confirmed that coagulation activation is an important link in the pathogenesis of sepsis, and intervention in coagulation function may alleviate inflammatory reactions in the body and improve the prognosis of patients. At present, the treatment methods for sepsis mainly include anti infection, volume resuscitation, organ function support, anticoagulation and blood sugar control, nutritional support, etc. The 2021 International Guidelines for Sepsis recommend the use of venous thromboembolism (VTE) drug prevention and low-molecular-weight heparin for adult sepsis/septic shock patients as long as there are no contraindications. Low molecular weight heparin is converted from ordinary heparin, with significant advantages such as high bioavailability, long half-life, low bleeding risk, and low cost. It can also quickly and continuously inhibit thrombosis.
Participant
Inclusion criteria (1) A prospective randomized controlled clinical study on LMWH treatment for Chinese sepsis patients published publicly; (2) Diagnostic criteria: Meets the diagnostic criteria for sepsis 3.0. (3) The research subjects are over 18 years old. Exclusion criteria (1) Pregnant or lactating patients; (2) Patients who have undergone organ transplant; (3) Those who have used anticoagulants 48 hours ago, have a history of coagulation dysfunction or abnormal coagulation; (4) There is a tendency for bleeding related to coagulation disorders (platelet count (PLT)<30 × 109/L); (5) Individuals with a history of severe brain injury, cerebral aneurysm, or vascular malformation; (6) Active bleeding (such as active upper digestive tract and urinary system bleeding); (7) Renal failure requires hemodialysis or hemofiltration;

Inclusion Criteria :lowmolecular-weight heparin administration given at any dose or frequency. route:subcutaneous injection Exclusion Criteria: Patients in the intervention and control arms both received therapeutic doses of lowmolecular-weight heparin (e.g.doses greater than needed for prevention of deep venous thrombosis).
Comparator Control
The control group included placebo treatment.
Main Outcome
28 day mortality rate, DIC incidence rate, MODS incidence rate, adverse reaction incidence rate. Measures of effect We expressed pooled continuous data as mean differences, with 95% CIs. Pooled dichotomous data were expressed as risk ratios or Peto odds ratio with 95% CIs.
