- Review Question
- What is the efficacy of use of cholinesterase inhibitors for reduction of cognitive adverse effects of electroconvulsive therapy?
- Study Type Included
- Randomized controlled studies with data available on outcomes related to memory/cognitive function are included. Animal studies, case reports, case series, systematic reviews, meta-analyses, commentary, conference abstracts, protocols without results, and studies only in abstract form are excluded.
- Condition Being Studied
- Electroconvulsive therapy (ECT) is effective for treatment-resistant depression, schizophrenia, and mania, but its use may be limited by adverse effects, including cognitive deficits. Cognitive adverse effects of ECT may be acute, subacute, or chronic. Adverse effects may include postictal confusion and delirium, nonmemory cognitive deficits, and retrograde and anterograde amnesia. Since cognitive adverse effects may limit tolerability of ECT, more information is needed about therapies that may potentially mitigate these effects.

- Patients undergoing electroconvulsive therapy and receiving donepezil, galantamine, or rivastigmine were included and compared to patients undergoing electroconvulsive therapy who did not receive a cholinesterase inhibitor.

- The theory that alterations in cholinergic neurotransmission may contribute to cognitive deficits associated with electroconvulsive therapy makes cholinesterase inhibitors a plausible pharmacologic treatment option. Cholinesterase inhibitors evaluated in this review include donepezil (doses of 5-23 mg orally daily or 5-10 mg/24-hour transdermal patch once weekly), galantamine (doses of 4-12 mg IR orally BID or 8-24 mg ER orally daily), and rivastigmine (1.5-6 mg BID orally or 4.6-13.3 mg/24-hr transdermal patch applied daily), which were administered to patients undergoing electroconvulsive therapy.
- Comparator Control
- Participants in the control group received placebo or an active comparator.
- Main Outcome
- Main outcome includes cognitive functioning after electroconvulsive therapy, as measured by memory/cognitive assessment scales. Scale used, scores obtained, and timepoints measured will be evaluated. There are no restrictions on the type of assessment scale or timeframe evaluated. Measures of effect Scores obtained on MMSE, WMS, PGI-MS, RBANS, ADAS-Cog, ACE-R, CGI, or other cognitive assessment scales used.

- Safety outcomes will include incidence of adverse effects associated with cholinesterase inhibitors (including but not limited to GI adverse effects and CNS adverse effects) when available. Measures of effect Safety outcomes will be measured using number of patients/incidence of adverse effect
