Cardiovascular outcomes in trials of cholinesterase inhibitors in people with Parkinson’s disease and other forms of Parkinsonism.
Participant
Any patient with one of the included conditions (Parkinsonism, Idiopathic Parkinson’s disease, parkinsonian disorders, Parkinson’s disease dementia, Lewy body dementia, Supranuclear Palsy, Multiple system atrophy, Corticobasal degeneration, atypical parkinsonism, Parkinson’s plus) were included in the review. There were no other restrictions on the participants.

Inclusion: Acetylcholinesterase Inhibitors, Cholinesterase Inhibitors, Rivastigmine, Donepezil, Galantamine, Exelon, Razadyne, Aricept, Evess Exclusion: Pyridostigmine Bromide, Neostigmine, Tacrine
Comparator Control
Inclusion: Placebo or other medication
Main Outcome
Incidence, classification and sequelae of CV AEs Cardiovascular adverse events occurring at any timepoint during trials of cholinesterase inhibitors in people with Parkinsons and related disorder. Cardiovascular adverse events will be defined using MedDRA coding as an event with a primary system organ class of either cardiac or vascular disorders. In situations where MedDRA classification was not used, CV AEs will be included and coded insofar as possible using the MedDRA classification. Synthesising data on incidence, classification and sequalae of cardiovascular events in this population will support clinicians in making informed decisions about prescribing in their clinical populations.

Additional Outcome
To compare using meta-analysis (or meta-regression if appropriate), the incidence rates between cholinesterase treated groups and placebo-control groups. To describe the approach to managing risk and monitoring for cardiovascular adverse events in these studies including inclusion/exclusion criteria and study methodology including risk profiling and monitoring. This can be used to help stratify high risk patients with pre-existing cardiovascular co-morbidities and how to best manage them in conjunction with cholinesterase prescribing.
