- Primary question : What is the relative efficacy of pharmacological agents to improve cognition in vascular dementia? Secondary questions: What is the efficacy of various pharmacological agents compared to placebo in the treatment of vascular dementia? What is the safety profile of various pharmacological agents compared to placebo in the treatment of vascular dementia with regards to treatment-related adverse events? What is the safety profile of various pharmacological agents compared to placebo in the treatment of vascular dementia with regards to serious adverse events? What is the safety profile of various pharmacological agents compared to placebo in the treatment of vascular dementia with regards to individual adverse events? What is the comparative efficacy and safety of pharmacological agents amongst themselves in the treatment of vascular dementia?
- Study Type Included
- We will include only Randomised Clinical Trials with comparator arms to assess the efficacy and safety of the treatment for vascular dementia.
- Condition Being Studied
- Pharmacological interventions used to manage Vascular dementia: Vascular dementia refers to changes to memory, thinking and behaviour resulting from conditions that affect the blood vessels in the brain. Cognition and brain function can be significantly affected by the size, location and number of vascular changes. It is a subset of Vascular Cognitive Impairment (VCI) which encompasses a range of cognitive deficits resulting from cerebrovascular disease, from mild cognitive impairment to full-fledged dementia. VaD arises from cerebral vascular insults, either ischemic or hemorrhagic in nature. Currently, there is no definitive treatment for Vascular dementia, and no regulatory bodies have approved medications.

- Inclusion: Patients diagnosed with vascular dementia (VaD)

- Intervention
- Participants receive the following drugs Cholinesterase inhibitors) : Donepezil Galantamine Rivastigmine Memantine Nootropics: Gingko, piracetam Ergot alkaloids: hydergine, nicergoline Xanthine derivatives: propentofylline, pentoxifylline, denbufylline Other drugs: Cerebrolysin, Cytidinediphosphocholine, Paracetamol, Actovegin An attempt will be made to include all drugs further identified from literature review on pharmacological interventions in vascular dementia.

- Cognition (ADAS-Cog, MMSE, etc.) Safety profile (Adverse events) Measures of effect Measures of effect: For Binary outcomes, Risk ratios(RRs) and Risk ratios with 95% confidence intervals (CIs) For Continuous outcomes, Mean Differences (MDs) with 95% confidence intervals (CIs). Differences in the cognition scores between populations will be extracted as mean differences. For cases where different instruments were used to measure the same continuous outcome, Standardised Mean Difference (SMDs) with 95% confidence intervals (CIs) Dichotomous Data will be converted to Continuous data based on feasibility. For time-to-event outcomes, Hazard ratios (HRs) and their corresponding 95% confidence intervals (CIs)
