A systematic review and meta-analysis to assess the effectiveness of vitamin D as an alternative to FDA-approved treatment and other therapies for managing vulvovaginal atrophy and sexual inactivity in postmenopausal women
Review Question
Could we consider vitamin D as an alternative therapy to other available therapies in the treatment and management of vaginal atrophy and sexual dysfunction in menopause and postmenopause?
Study Type Included
We will include randomized controlled trials (all RCTs in any language) that deal with the effect of different routes of administration of vitamin D (orally or topically, such as vaginal suppositories or vaginal cream) in managing vulvovaginal atrophy, vaginal PH, dyspareunia, sexual inactivity, and dysfunction, and on improving the vaginal maturation index and female sexual function index in menopausal and postmenopausal women.
Condition Being Studied
Vaginal atrophy, vulvovaginal atrophy, vaginal dryness, dyspareunia, and sexual inactivity and dysfunction as major symptoms of menopause and postmenopause
Participant
1. 45 years old and above healthy menopausal /postmenopausal women 2. Breast Cancer menopausal /postmenopausal females who previously had taking radiotherapy or chemotherapy or hormonal /Endocrine such as; tamoxifen or Aromatase inhibitors (AIs) or any other hormonal therapy which decreases estrogen level and causes a genitourinary syndrome of menopause (GSM) mainly “Vaginal atrophy “, vaginal dryness as a major side effect. 3. Menopausal Women who have a history of cancer, or who are at high risk of cancer, or have current mild/moderate stages of breast cancer 4. Menopausal/postmenopausal women who have cardiovascular disease (CVD) or at moderate or high risk of CVD or who have venous thromboembolism (VTE) or a history of VTE 6. Menopausal/ postmenopausal women who have urinary incontinence ( stress incontinence ) 7. Menopausal/postmenopausal women who have (GSM) particularly, Vulvovaginal atrophy and sexual inactivity and dysfunction 8. Menopausal/postmenopausal women who are sexually active and/or have low female sexual function index scale ( FSFI ) </=26.55, and who have a low vaginal health index Exclusion criteria: 1. Women younger than 40 years 2. Women have a recurrent vaginal infection or urinary tract infections (UTI) 4. Observational studies, reviews, Post hoc studies, Duplication publication

Intervention
Vitamin D (cholecalciferol D3 or calcitriol, alpha-calcidiol, or ergocalciferol D2) in different dosage forms and routes of administration (oral supplements, injections, or topical forms such as vaginal suppositories, creams, or gels) alone or in combination with other therapies (Ca2+, Raloxifene, isoflavone, and others)
Comparator Control
Different comparators: Placebo or control group or HRT ( Hormonal replacement therapy such as Estrogen and/or progestogen ), or other drugs such as Raloxifene or Ca 2+ or combined estrogen
Main Outcome
Evaluate the improvement in VMI ( vaginal maturation index)
