Assessment of biological carotenoid levels as biomarkers of dietary intake: a systematic review and meta-analysis protocol
Review Question
This systematic review aims to provide a comprehensive updated synthesis on relevant articles that compared biological assessment (blood, skin and other sources) of carotenoids (total carotenoids, α-carotene, β-carotene, cryptoxanthin, lutein, lycopene, and zeaxanthin) with their respect assessment of dietary intake (food frequency questionnaire, dietary recall, diet record, diet history, weighed food record, dietary intervention, other methods). Ultimately, our aim is to provide evidence-based guidance to inform if the biological measurement of carotenoids represents a reliable and replicable biomarker of their dietary intake in the general population.
Study Type Included
Randomised controlled trials, observational studies (all types), longitudinal studies, cohort studies, case-control studies, cross sectional studies or before after studies will be included in this review.
Condition Being Studied
Assessment of dietary intake is known to involve a number of challenges and limitations, particularly related to the subjectivity of dietary collection. A reliable, accurate and replicable method of dietary intake, particularly looking at the consumption of key nutrients present in healthy diets, such as fruits and vegetables, would provide enormous advantages to the field of nutrition and health. Carotenoids are highly prevalent in these foods groups and are considered of interest as a marker of their regular dietary consumption.
Participant
Only studies on adult healthy population (≥ 18 years old) will be included in the systematic review.

Intervention
Studies will be eligible if they evaluated biological assessment of carotenoids (total carotenoids, α-carotene, β-carotene, cryptoxanthin, lutein, lycopene, and zeaxanthin) identified in either blood, skin or other sources.
Comparator Control
Studies with the following comparator groups will be eligible: measures of carotenoids from dietary intake (food frequency questionnaire, dietary recall, diet record, diet history, weighed food record, dietary intervention, and any other form of dietary assessment). Studies had to report the comparison or association/correlation between levels of measured carotenoids and their composition estimated through method of dietary intake.
Main Outcome
The primary outcome of the systematic review is to assess the validity of carotenoids as a biomarker to measure dietary intake, while the secondary outcomes are: to identify the most reliable method (s) of dietary collection and biological assessment of carotenoid (blood vs skin vs other sources); to identify which of the measured carotenoids (total carotenoids, α-carotene, β-carotene, cryptoxanthin, lutein, lycopene, and zeaxanthin) represents the strongest association with dietary assessment; and to identify optimal timing to use carotenoids as reflection of dietary intake e.g. 24hrs vs 1 week. Measures of effect Effect of the primary and secondary outcomes will be expressed either as odds ratios and risk ratios or mean difference and standardised mean difference.
