- Title
- Colchicine for the Treatment of Pericarditis: A Meta-Analysis of Randomized Controlled Trials
- Review Question
- How does cholchine for the treatment of pericarditis: A meta-analysis of randomized controlled trials
- Study Type Included
- The methods used in this study were developed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines.
- Condition Being Studied
- Pericarditis. Pericarditis is the condition where the pericardial layers become inflamed due to various triggers that cause a specific immune response. Pericarditis is the type of pericardial disease that mostly happen and primarily affects individuals that are young or middle-aged and more susceptible to men rather than women. There was a incidence of 27.7 cases per 100, 000 population each year reported to recurs
- Participant
- Inclusion : This study includes criteria of the following: 1. Patient with recurrent pericarditis that were diagnosed by : • A first episode of acute idiopathic pericarditis • An interval of 6 weeks or longer with no symptoms • Recurrence of pericarditis that were characterized by recurrent typical pericarditic chest pain and 1 or more of the following signs: pericardial friction rub, evolving ECG changes, or echocardiographic evidence of a new pericardial effusion. 2. Pericarditis patients with no previous history of colchicine consumption 3. Studies that compare colchicine with placebo or any conventional non-steroidal anti-inflammatory drug (NSAID) in treating patient with pericarditis 4. Studies that showed the recurrence of pericarditis incidence in the outcome Exclusion : 1. Population of patient with (1) tuberculous, neoplastic, bacterial/purulent pericarditis; (2) severe liver disease that’s characterized by the increased of aminotransferase level greater than 1.5 times upper limit of normal range; (3) increase of serum creatine level above upper limit of normal range 2. Clinical trial studies that were not randomized or non-placebo controlled randomized clinical trial 3. Observational non-cohort study, case report, review article
- Animal
- Human Disease Modelled
- Intervention
- The intervention group is administration of colchicine in any dose compared to control group which is placebo. Placebo was used as the comparator because anti-inflammatory treatment alone was a standard treatment. The standard treatment consists of aspirin, ibuprofen and indomethacin. The patients also receive a proton pump inhibitor as gastroduodenal prophylaxis.
- Comparator Control
- Placebo
- Main Outcome
- The use of colchicine as adjunctive therapy to the conventional one has shown effectiveness in decreasing the frequency of recurring pericarditis and it can prolong the recurrence duration to happen. Measures of effect Colchicine intervention can reduce the recurrence of pericarditis with the RR was 0.53 (CI95% 0.40, 0.71; P<0.0001), and the mean differential duration to the first recurrence is 1.69 (CI95% 1.13, 2.25; P<0.00001) months. The heterogeneity test showed homogenous data in both primary (I²=58%, P = 0.09) and secondary analysis (I²=0%, P = 0.56).
