- Review Question
- Does calcium phosphate-based composites are effective for direct pulp capping in animal models, compared to capping agent controls? Context and rationale Pulpal direct exposure to the oral environment breaks the integrity of the pulp-dentin complex and, if not properly treated, can cause apical periodontitis, leading to endodontic treatment or extraction. Direct pulp capping (DPC) is the procedure in which the exposed pulp is covered with a biocompatible material, applied directly to the exposed area. This treatment aims to maintain pulp vitality and health by sealing the pulp against bacteria and inducing dentin bridge formation at the site of exposure.

- Inclusion criteria: Experimental animal studies, including Sprague-Dawley and Wistar rats, which were submitted to mechanical pulp exposure. Exclusion criteria: Human studies, such as randomized trials. Ex vivo, in vitro and in silico studies will also be excluded

- Inclusion criteria: Direct pulp capping performed using calcium phosphate-based composites Exclusion criteria: Interventions such as indirect pulp capping, pulpotomy and studies that analyzed materials that did not include calcium phosphate in their composition, or used the material for another purpose
- Comparator Control
- Inclusion criteria: Calcium hydroxide (CH) and mineral trioxide aggregate (MTA) are the most commonly used materials as control group in studies related to direct pulp capping. But all studies, regardless of the type of control group used, will be included. Exclusion criteria: None

- Inclusion criteria: Histological analyzes based on the studies by Six et al. 2002, with modifications, are frequently performed studies about this topic in order to determine the antimicrobial properties, inflammatory response, as well as the ability to induce the dentin barrier. (1) Reparative dentin formation: 0=no hard tissue formation 1=incomplete hard tissue formation 2=thick hard tissue formation; (2) Pulpal inflammation: 0=none of a few scattered inflammatory cells beneath the pulp exposure site 1=mild inflammatory cells, such as mono- or polymorphonuclear leukocytes 2=moderate inflammatory cell infiltration involving the third coronal radicular pulp 3=severe inflammatory cell infiltration involving the third coronal or more radicular pulp; (3) Tissue necrosis: 0=normal tissue beneath the pulp exposure site 1=odontoblast-like cells, odontoblasts, and pulp tissue pattern disorganization or odontoblast hyperactivity, but normal central pulp tissue pattern 2=general disorganization of the pulp tissue pattern 3=pulp necrosis and (4) Bacterial infiltration: 1- Absence of stained bacterial profiles in any of the sections (none). 2- Presence of stained bacterial profiles along the coronal or apical walls of the cavity (mild). 3- Presence of stained bacterial profiles within the cut dentinal tubules or axial wall of the cavity (moderate). 4- Presence of stained bacterial profiles within the dental pulp (severe) Exclusion criteria: Studies that did not evaluate the criteria mentioned above
