Perioperative Thromboprophylaxis in Upper Extremity Replantation: A Systematic Review
Review Question
What is the efficacy of various types of perioperative anticoagulation protocols on upper extremity replantation?
Study Type Included
We will include randomized controlled trials, nonrandomized controlled trials, cohort studies, case–control studies, and case series to assess the various perioperative thromboprophylaxis protocols for upper extremity replantation and their success with survival of the limb/digit. We will exclude gray literature like expert opinion and single case report, unpublished literature, animal studies, and studies >40 years old.
Condition Being Studied
Upper extremity replantation and survival with type of thromboprophylaxis protocol used perioperatively.
Participant
Inclusion: humans of any age that underwent an upper extremity replantation where microvascular anastomosis occured Exclusion: animals

Intervention
Perioperative thromboprophylaxis includes local and systemic administration of thromboprophylactic agent and all dose regimens, as well as intra-operative hemodynamic/anesthesia recommendations and postoperative care for thromboprophylactic measures with the aim of successful take of an upper extremity replantation. Examples include lidocaine, heparin, aspirin, dextran, phosphodiesterase inhibitors, hemoprophylaxis, systemic vasodilators, as well as regional anesthesia and blood pressure parameters.
Comparator Control
Not applicable given this is a systematic review focused on the various protocols that exist surrounding perioperative thromboprophylaxis for the successful take of upper extremity replantation.
Main Outcome
The primary outcome measured will be success of upper extremity replantation defined by replantation survival which is an indication of microvascular patency from the anastomosis intervention. Measures of effect mean replant survival rate, % survival

Secondary outcomes measured will be intervention-related complications and average hospital stay.
