- Title
- Effects of levothyroxine therapy on bone metabolism in hypothyroidism patients: a systematic review and meta-analysis
- Review Question
- Does levothyroxine replacement therapy have significant effects on bone metabolism in hypothyroidism patients?
- Study Type Included
- Randomized controlled trials and observational studies.
- Condition Being Studied
- Hypothyroidism, comprising both subclinical hypothyroidism and overt hypothyroidism. Bone metabolism status assessed by serial measurements of bone mineral density (BMD) , calcium, phosphorus, bone alkaline phosphatase (BALP) , N-terminal propeptide of type I procollagen (PINP) and C-telopeptide of type I collagen (CTX-I).
- Participant
- Inclusion: individuals who was diagnosed as overt hypothyroidism or subclinical hypothyroidism. Exclusion: (1) patients with congenital hypothyroidism, central hypothyroidism, or pregnant woman; (2) studies included participants younger than 18 years old; (3) studies included patients who suffered from thyroid cancer or received thyroidectomy.
- Animal
- Human Disease Modelled
- Intervention
- Inclusion: patients who received levothyroxine (LT4) treatment only without any restriction on the dosage or duration. Exclusion: (1) patients received LT3 monotherapy or LT3+LT4 therapy; (2) patients were taking drugs that affected bone metabolism (e.g. bisphosphonates, estrogens, parathyroid hormone etc.) at any time point during observation time.
- Comparator Control
- Inclusion: (1) patients received placebo or without treatment; (2) euthyroid patients. Exclusion: self control.
- Main Outcome
- Bone mineral density (BMD) Measures of effect Standardized mean difference (SMD) or weighted mean difference (WMD)
- Outcome Measure
- Additional Outcome
- Calcium, phosphorus, bone alkaline phosphatase (BALP), N-terminal propeptide of type I procollagen (PINP) and C-telopeptide of type I collagen (CTX-I). Measures of effect Standardized mean difference (SMD) or weighted mean difference (WMD)
