Title
Systematic review and meta-analysis on Epinephrine Use in Out-of-Hospital Cardiac Arrest: An Examination of Clinical Outcomes and Safety.
Review Question
Impact of epinephrine administration on survival and neurologic outcomes in out-of-hospital cardiac arrest patients, its limitations and potential harms associated with this intervention.
Study Type Included
Types of studies to be included: Randomized controlled trials (RCTs), observational studies that investigate the use of epinephrine for cardiac arrest treatment in out-of-hospital settings. Types of studies to be excluded: Studies that focus on the use of epinephrine for cardiac arrest treatment in hospital settings or during surgical procedures, and do not report relevant outcomes or have incomplete data.
Condition Being Studied
Condition/Healthcare Domain: Cardiac Arrest The study falls within the domain of emergency medicine and resuscitation. It focuses on the use of epinephrine, a medication commonly used in emergency care settings, for the treatment of cardiac arrest that occurs outside of the hospital. This research is relevant to pre-hospital care providers, emergency medical services (EMS) personnel, and healthcare professionals involved in managing cardiac arrest events in out-of-hospital settings, with the goal of improving patient outcomes and informing evidence-based practices for emergency care.
Participant
Participants/population: Adults or aged or young adults or middle aged Inclusion criteria: Studies conducted on human participants, focusing on out-of-hospital cardiac arrest (OHCA) patients, evaluating the use of epinephrine in the management of OHCA. Exclusion criteria: Studies conducted on animals or in vitro models, not focusing on OHCA patients, not evaluating the use of epinephrine, without clinical outcomes or safety data available.
Animal
Human Disease Modelled
Intervention
Intervention(s), Exposure(s): Epinephrine or adrenaline or Racepinephrine The intervention of interest in this systematic review and meta-analysis is the use of epinephrine in the management of out-of-hospital cardiac arrest (OHCA) patients. Epinephrine, also known as adrenaline, is a medication commonly used during cardiopulmonary resuscitation (CPR) to stimulate the heart and increase blood flow to vital organs in an attempt to improve survival outcomes in OHCA patients. Exposures include the administration of epinephrine via different routes (e.g. intravenous, intraosseous, endotracheal), different dosages, and at different time points during the management of OHCA. This may also include comparisons between epinephrine and other interventions or medications used in the management of OHCA, such as placebo or alternative vasopressors.
Comparator Control
Non-use of prehospital epinephrine, achievement of pre-hospital Return of spontaneous circulation(ROSC)
Main Outcome
Return of spontaneous circulation (ROSC) achieved pre-hospital, Survived to hospital discharge, CPC 1 (normal function) or 2 (mild to moderate disability) Measures of effect Relative Risks, Odds ratios
