- Condition Being Studied
- Subclinical hypothyroidism (SCH) is a biochemical condition defined by elevated serum thyroid stimulating hormone (TSH) in the setting of normal levels of thyroid hormones. SCH is associated with cardiovascular disease risk factors such as elevated blood pressure.
- Participant
- Inclusion criteria: 1. Clinical trial studies (RCTs) that investigated the effect of levothyroxine (LT4) treatment on blood pressure or prospective observational studies that compared blood pressure levels before and after levothyroxine treatment. 2. Patients with SCH who are over 18 years old (adults). 3. Treatment intervention is only with levothyroxine. 4. The compared group in RCT studies should be the group with SCH not receiving treatment with LT4 or receiving a placebo. In prospective observational studies, blood pressure before treatment should be compared. 5. The included studies should show the difference in systolic blood pressure (SBP) or diastolic blood pressure (DBP) between groups treated with levothyroxine and control groups without levothyroxine treatment (RCT studies) or changes in SBP or DBP levels before and after treatment reported with LT4 (prospective observational studies). Exclusion criteria: 1. Patients under 18 and pregnant women 2. Using the combination of LT4 with other drugs as the therapeutic intervention 3. Studies without usable data on the effect of LT4 therapy on blood pressure 4. Studies containing overlapping data

- The intervention is thyroid hormone replacement therapy with levothyroxine, the most used drug for thyroid hormone supplements. The combined use of levothyroxine and other medications as the intervention will be excluded.
- Comparator Control
- The control group for RCTs is placebo or not receiving levothyroxine. The comparators for prospective observational studies are patients with SCH before starting the treatment.
- Main Outcome
- The Main outcome is the difference in systolic and diastolic blood pressure between levothyroxine-treated groups and controls in RCTs. Also, in the prospective observational studies, the outcome is the change in systolic and diastolic blood pressure levels before and after levothyroxine treatment. The mean differences of systolic and diastolic blood pressure will be pooled using meta-analysis. Measures of effect Mean differences (95% Confidence Interval)
