The efficacy of acetic acid therapy in the management of pseudomonas aeruginosa related flap infection in oral & maxillofacial surgery
Review Question
The efficacy of acetic acid in the management of pseudomonas aeruginosa related flap infection in oral & maxillofacial surgery
Study Type Included
Inclusion criteria: 1. RCTs and case series investigating the efficacy of acetic acid therapy in the management of pseudomonas aeruginosa related flap infection in oral & maxillofacial surgery 2. English language 3. Human subjects Exclusion criteria: 1. Non-English 2. Case reports, abstracts, book chapters, oral/poster presentations 3. Papers where acetic acid therapy is not utilised
Condition Being Studied
Pseudomonas aeruginosa is an opportunistic pathogen with an innate resistance to many antibiotics & disinfectants. It is also considered by many as an important cause of nosocomial infections due to its ability to survive in the hospital environment. It has also been associated with an increase in hospital stay, an increase in cost, and also with high morbidity. Oral and maxillofacial surgical patients that have undergone free flap surgery are susceptible to such infections due to longer hospital stay. If flap recipient site becomes infected, the patient may be at an increased risk of return to theatre, infection of underlying metal-work (if present) and flap failure. This introduces a higher degree of morbidity to this patient cohort.
Participant
Patients: Oral and maxillofacial patients that have undergone free flap surgery and that have developed pseudomonas aeruginosa infection in the recipient site
Animal
Human Disease Modelled
Intervention
Acetic acid therapy
Comparator Control
Alternative management options of pseudomonas aeruginosa infected flaps, e.g. washout with betadine & saline, antibiotics, regular dressing changes
Main Outcome
1. Clearance of pseudomonas aeruginosa infection Measures of effect Relative risk and odds ratio
Outcome Measure
Additional Outcome
1. Length of hospital stay in each treatment group 2. Number of times patients return to theatre 3. Flap failure 4. Number of washouts Measures of effect 1. Mean difference between treatment groups (with regards to length of hospital stay, return to theatre, and number of washouts) 2. Relative risk and odds risk for flap failure
