Condition Being Studied
Sepsis is a condition characterized by life-threatening organ dysfunction resulting from a dysregulated host response to infection [1]. In 2017, the Global Burden of Disease Study reported an estimated 48.9 million cases of sepsis (with a 95% uncertainty interval of 38.9-62.9 million), leading to 11.0 million deaths related to sepsis (within a range of 10.1-12.0 million) and these sepsis-related fatalities accounted for approximately 19.7% of all global deaths. Nevertheless, the impact of adjunctive hydrocortisone on short- or long-term mortality remains a topic of debate, possibly due to variations in treatment regimens. Some randomized clinical trials (RCTs), like the VANISH trial and ADRENAL trial, exclusively employed hydrocortisone, while other studies utilized a combination of hydrocortisone and fludrocortisone. However, some studies have not observed a clinically significant benefit of hydrocortisone-fludrocortisone combination therapy for treating septic shock. Therefore, this systematic review and meta-analysis were conducted to elucidate the effect of hydrocortisone-fludrocortisone combination therapy in patients with septic shock, with a particular focus on mortality.
