- Salvage treatment for sudden sensorineural hearing loss with intratympanic(IT) steroids : a systematic review and network meta-analysis
- Review Question
- We conducted a systematic review incorporating Network Meta-Analysis (NMAs) to assess the relative effects of different therapeutic schedules of intratympanic (IT) steroids, to evaluate the efficacy, safety and other indexes of them, to find the optimal medications, dosage, concentrations, timing, and administration schedules for IT therapy.
- Study Type Included
- We will include randomized controlled trials (RCT) to assess the beneficial effects of the treatments.
- Condition Being Studied
- Sudden sensorineural hearing loss (SSNHL) is a subset of Sudden hearing loss (SHL) that (a) is sensorineural in nature, (b) occurs within a 72-hour window, and (c) consists of a decrease in hearing of 30 decibels affecting at least 3 consecutive frequencies. The standard treatment of SSNHL constitutes systemic steroid. However, 30-50% of patients display a more treatment refractory course. For these patients, intratympanic (IT) steroids therapy as a recommendation for salvage treatment. What puzzled us was that there are many kinds of steroid choices and ways of administration. Which one is the best? Unfortunately, there is currently a lack of high-quality medical evidence-based evidence. So, we thus a network meta analysis to achieve these comparison ns.

- Inclusion criteria : hearing loss (SSNHL) administering intratympanic (IT) steroids therapy offered after 2 weeks from symptom onset (even if initial therapy was observation) Exclusion criteria: Patients with sudden hearing loss such as vestibular schwannoma, Meniere's disease, and Lyme disease were excluded.

- The intratympanic (IT) steroids therapy as a salvage treatment. The different modes of administration in intratympanic steroids: the tympanic membrane injection; the perfusion via ventilation tube; the perfusion via the round window catheter; the perfused via the eustachian tube; Sealing of the round and oval window niches with fascia The different steroids choices for treatment: dexamethasone, triamcinolone, hydrocortisone, betamethasone, prednisone, cortisone, prednisolone, methylprednisolone

- mean hearing gain [i.e., pure tone audiometry, speech recognition scores (including word recognition and speech discrimination scores, and speech reception threshold (SRT)]

- Extent of recovery (e.g., Sieigel’s Criteria, or other such endpoint measures that categorize patients’ recovery as complete, marked, slight, or none based on decibels of improved hearing); harms (e.g., withdrawals due to adverse effects, otitis media, residual tympanic membrane perforation);incidence of vestibular endpoints (e.g., vertigo); reduction of tinnitus [could be measured via different techniques such as psychoacoustic tests of tinnitus (e.g., pitch match, loudness match, maskability, residual inhibition), rating scales (e.g., verbal rating scale, numerical rating scale, visual analog scale, poster style, mechanical device, etc.), questionnaires describing functional effects (e.g., tinnitus questionnaire, tinnitus handicap questionnaire, tinnitus severity scale, subjective tinnitus severity scale/tinnitus reaction questionnaire, tinnitus severity grading, tinnitus severity index, tinnitus handicap inventory, intake interview for tinnitus retraining therapy), and patients’ global perception of treatment-related changes).
