- Sex Hormones and Pain: A Systematic Review and Meta-analysis.
- Review Question
- This review will investigate the relationship between sex hormones and pain in humans, including (1) the correlation between endogenous sex hormones and pain and (2) the role of exogenous sex hormones on pain.
- Study Type Included
- Studies that meet the following criteria will be included: (1) the results were published in a peer-reviewed journal; (2) The subjects were humans (older than 18 years), including either patients with clinical pain conditions or healthy participants; (3) valid measurements of levels of sex hormones (i.e., estradiol, progesterone, or testosterone); (4) valid measurements of pain-related outcomes (i.e., pain threshold, pain tolerance, conditioned pain modulation, pain-intensity, and unpleasantness ratings); (5) the results reported the relationship between sex hormones and pain. Studies involving the following criteria will be excluded: (1) review articles, dissertations, patents, book reviews, case studies, or animal studies; (2) subjects with primary symptoms other than pain; (3) subjects with hormonal disorders or being transgender; (4) studies did not report main outcomes of pain; (5) studies did not involve any type of sex hormone (i.e., estradiol, progesterone, or testosterone).
- Condition Being Studied
- Pain includes experimental pain and clinical pain conditions (primary symptoms). Sex hormones include estrogen, testosterone, and progesterone.
- Participant
- Human participants (older than 18 years) will be studied, including healthy samples and patients with clinical pain conditions.

- Intervention
- The interventions are defined as those that expose participants to either the administration of sex hormones or sex hormone therapy. It also can be included if participants are exposed to both pain measurements and sex hormone measurements. The sex hormones include estradiol, progesterone, and testosterone.
- Comparator Control
- The control group involving an active control intervention (e.g. a placebo or another intervention) and a passive control intervention (e.g. no intervention or a waiting list) will be included.
- Main Outcome
- The primary outcomes are both pain-related outcomes (e.g., pain threshold, pain tolerance, conditioned pain modulation, pain intensity, and unpleasantness ratings) and sex hormone levels (i.e., estradiol, progesterone, or testosterone) in both healthy samples and patients with clinical pain conditions. Apart from the outcomes mentioned above, in the patient samples, the dosage of analgesic drug used also be considered as the primary outcome of pain. It should be noted that in migraine patients, the frequency and duration of headache attacks are also considered as primary outcomes.
