- Title
- Evaluating the Role of Traditional Chinese Herbal Laxatives for Bowel Preparation in Colonoscopy:A Systematic Review and Network Meta-analysis.
- Review Question
- Evaluating the role of traditional chinese medicine such as senna leaf for bowel preparation in colonoscopy
- Study Type Included
- We will only include parallel-group randomized trials. We will include studies regardless of their publication status or the country of origin. We will not consider cross-over or cluster-randomized controlled trials, since these are not applicable to the clinical question of interest.
- Condition Being Studied
- Colorectal cancer(CRC)is one of the common gastrointestinal cancers, its morbidity and mortality rate continue to rise, making it the second highest incidence tumor in China after lung cancer. The occurrence and development of colorectal cancer mostly follow the sequence of "adenoma-cancer". It generally takes 5-10 years to develop from precancerous lesions to cancer, which provides an important time for early diagnosis and clinical intervention. In China, more than 85% of colorectal cancers are found in the advanced stage. Even with comprehensive treatment such as chemotherapy and radiotherapy, the survival rate of patients does not exceed 40%. After treatment of early-stage colorectal cancer, the 5-year survival rate can exceed 95%. Therefore, early diagnosis and treatment of colorectal cancer are crucial to reduce the mortalityrate of colorectal cancer.Early screening methods for colorectal cancer include colonoscopy, FIT, mtsDNA, Septin9, colon CT imaging, capsule colonoscopy. Among them, colonoscopy is the gold standard for colorectal cancer screening. Its CRC detection rate is 100%, and it can reduce the long-term incidence of CRC by 31-71% and the long-term mortality by 26%. It is a diagnostic and therapeutic technique that can detect cancer and precancerous lesions. The review interval can be as long as 10 years.
- Participant
- Inclusion criteria: Patients aged ≥18 years who require colonoscopy, no matter in outpatient clinics or wards. All patients signed the informed consent form. Exclusion criteria: refusal of colonoscopy, previous partial colectomy, planned polypectomy, incomplete bowel preparation, renal failure, massive ascites, or known colonic obstruction and so on.
- Animal
- Human Disease Modelled
- Intervention
- Bowel preparation using Chinese herbal laxatives, such as senna leaf, rhubarb, glauber and so on.
- Comparator Control
- Bowel preparation using polyethylene glycol or other non-Chinese herbal laxatives, such as dimethyl silicone oil, lactilose, compound sodium picosulfate, olive oil and so on.
- Main Outcome
- Boston bowel preparation scale(continuous outcome): BBPS, segment scores defined as follows. Score 0: unprepared colon segment, mucosa not seen due to solid stool that cannot be cleared. Score 1: portion of mucosa of colon segment seen, but other areas of the segment not well seen due to staining, residual stool and/or opaque liquid. Score 2: minor amount of residual staining, small fragments of stool and/or opaqueliquid, but mucosa of segment seen well. Score 3: entire mucosa of segment seen well with no residual staining, small fragments of stool and/or opaque liquid. Overall Adenoma detection rate(dichotomous outcome):defined as the proportion of patients with at least one adenoma, is one of the most important quality indicators in colonoscopy.
- Outcome Measure
- Additional Outcome
- Cecal intubation rate(dichotomous outcome):defined as insertion beyond the ileocecal valve with visualization of the appendix opening. Overall Adenoma detection rate(dichotomous outcome): defined as the proportion of patients with at least one adenoma, is one of the most important quality indicators in colonoscopy. Cecal intubation time(continuous outcome): defined as time for passage of the colonoscope from the rectum to the cecum. A stopwatch was used to time the procedures. Recent studies have shown that it is also an important quality indicators in colonoscopy.
