Review Question
How does sinomenine preparation impact on inflammatory arthritis compared to western medicine such as nonsteroidal anti-inflammatory agents (NSAIDs), slow-acting anti-rheumatic drugs (DMARDs), and biological agents?
Study Type Included
Only clinical randomized controlled trials (RCTs) will be included. Animal experimental studies, case reports, conference articles, expert experience, and reviews will be excluded.
Condition Being Studied
Inflammatory arthritis (IA) is a group of chronic inflammatory condition that can cause significant disability and joint pain, mainly including rheumatoid arthritis (RA), ankylosing spondylitis (AS), psoriatic arthritis (PsA), etc. IA is characterized by autoimmune-mediated joint inflammation and is associated with inflammatory cytokine networks. Despite common features such as inflammatory arthritis, these diseases differ in serological findings; patterns of disease, including axial skeletal involvement; and demographic features. Drug therapies used in inflammatory arthritis aim to reduce symptoms and suppress inflammation, joint damage and disability. Although drug treatment is important, non-drug therapies, which span advice, exercise, physiotherapy and surgery, all form crucial additional components of disease management. Sinomenine (SIN) is a herbal extracts of Caulis Sinomenii, used to treat inflammatory arthritis. Numerous basic experiments have confirmed its anti-inflammatory molecular mechanism. The purpose of this review was to systemically evaluate all randomized clinical trials (RCTs) of SIN for treating arthritis symptoms and to elucidate the efficacy of SIN for alleviating the symptoms of IA.
Participant
Inclusion: participants should be clinically diagnosed with IA (including RA, AS, GA) according to the diagnostic criteria, clinical guidelines; Exclusion: (1) patients who had conditions that would prevent the use of treatment protocols such as abnormal liver or renal function, history of peptic ulcer, allergy to SIN, or other drugs used in the study such as ibuprofen, diclofenac sodium, methotrexate and leflunomide. (2) patients with other autoimmune diseases or other serious conditions that could influence the results, such as severe heart failure, cancer, DIC, and severe infections. (3) using corticosteroids with doses more than 10 mg/day during the preceding 3 months, history of psychological disorders, and intraarticular injections during the preceding 3 months. (4) pregnancy or lactation.

Intervention
The intervention group received sinomenine preparation alone or sinomenine preparation combines with oral western medicine treatment, such as NSAIDs, DMARDs, and biological agents.
Comparator Control
Intervention with non sinomenine preparations (e.g. placebo, oral western medicine treatment, such as NSAIDs, DMARDs, and biological agents).
Main Outcome
The primary outcomes of RA include symptom evaluation (including the number of swelling joints affected by RA; the number of painful joints affected by RA; the duration of morning stiffness) and ACR20. The primary outcomes of AS are recognized standardized scales: bath ankylosing spondylitis disease activity index (BASDAI); bath ankylosing spondylitis functional index (BASFI); ASAS-endorsed disease activity score(ASDAS). The primary outcomes of GA include the serum uric acid (SUA), C-reactive protein (CRP), visual analogue scale (VAS), erythrocyte sedimentation rate (ESR), interleukin-6 (IL-6), interleukin- 8 (IL-8), and tumor necrosis factor-α (TNF-α). Measures of effect For continuous data, the results will be presented as standardized mean differences with 95% CIs. For dichotomous outcomes, data will be expressed as risk ratios with 95% CIs.

Additional Outcome
The secondary outcome of RA are clinical effectiveness, Rheumatoid factor (RF), Erythrocyte sedimentation rate (ESR), C-reactive protein (CRP) and adverse effects (AEs). The secondary outcome of AS include clinical symptom measures (morning stiffness time, the Schober test, chest expansion, occipital wall test, finger-to-ground distance, and 15 m walking time); laboratory measures (ESR and CRP); effective rate and AEs. The secondary outcomes of GA include the total effective rate and AEs. Measures of effect For continuous data, the results will be presented as standardized mean differences with 95% CIs. For dichotomous outcomes, data will be expressed as risk ratios with 95% CIs.
