Systematic review on efficiency and safety of classical herbal formulas with conventional western medicine in the treatment of ascites induced by liver cirrhosis.
Review Question
This study aims at comparing the efficacy and safety of various classical herbal formulas combined with conventional western medicine in the treatment of ascites due to HBV-related cirrhosis.The (Patient/Intervention/Comparison/Outcome/Study design) PICOS framework was employed as our eligibility criteria. These studies will be included if they were in accordance with the following criteria: 1)Population: met the recognized diagnostic criteria of cirrhosis ascites, simultaneously the condition of HBV positivity or HBV infection history. 2)Intervention: classical herbal formulas combined with conventional western medicine. Classical herbal formulas will include Yiguan decoction, Fangji-Huangqi decoction, Danggui Shaoyao powder, Fuling Guizhi Baizhu Gancao decoction, Zhenwu decoction, ShiPi decoction, Zhuling decoction, and Wuling powder. Conventional western medicine mainly refers to western medicine, including salt-restricted diet, diuretics, albumin, anti-HBV drugs, et al. 3)Control:conventional western medicine, including salt-restricted diet, diuretics, albumin, anti-HBV drugs, et al. 4)Outcomes: availability of quantitative data about ascites induced by HBV-related cirrhosis. 5)Study design:randomized controlled trials (RCTs).

Only randomized controlled trials (RCTs) were taken into our consideration.
Condition Being Studied
Liver cirrhosis is the final stage of liver fibrosis and has become a major health-related concerns worldwide. According to global reports, cirrhosis results in about one million deaths annually. The etiologies of liver cirrhosis include excessive alcohol consumption, viral hepatitis, non-alcohol-related fatty liver disease, autoimmune liver disease, and metabolic liver disease. But in China, hepatitis B virus (HBV) is the major cause. In recent decades, despite the improvement of antiviral therapy, the burden of viral hepatitis has remained considerable. Ascites is one of the most common complications of cirrhosis, the incidence approximately 5% to 10% per year. Current treatments focus on salt-restricted diet, diuretics, large volume paracentesis, transjugular intrahepatic portosystemic shunt, even liver transplantation. However, diuretics use is associated with an increased risk of hepatic encephalopathy and renal impairment. Hence, effective treatments with fewer complications are urgent to improve survival and health-related quality of life. Classical herbal formulas derive from ancient chinese medicine classic books and have still been used in clinical practice in China. Owing to relatively fixed and precise formula, these have become important sources of modern chinese medicine clinical prescription and chinese patent medicine development.
Participant
There patient will be included: met the recognized diagnostic criteria of cirrhosis ascites, simultaneously the condition of HBV positivity or HBV infection history.There patient will be excluded:cirrhosis ascites caused by alcohol consumption, autoimmune liver disease, metabolic liver disease, et al; the patients with other complications, such as gastrointestinal bleeding, spontaneous peritonitis, hepatorenal syndrome, et al.

Intervention
Interventions:classical herbal formulas combined with conventional western medicine.Classical herbal formulas will include Yiguan decoction, Fangji-Huangqi decoction, Danggui Shaoyao powder, Fuling Guizhi Baizhu Gancao decoction, Zhenwu decoction, ShiPi decoction, Zhuling decoction, and Wuling powder. Main Outcome
Clinical efficacy: According to the standard of the “Guidelines for the diagnosis and treatment of cirrhotic ascites and related complications in beijing 2017 ” or the “An expert consensus on TCM for diagnosis and treatment of cirrhotic ascites ”, the degree of ascites was rated as “markedly effective” , “effective” , or “ineffective”.Total effective rate=(“markedly effective” patients + “effective” patients)/all patients × 100%. Serum parameters for liver function: alanine aminotransferase (ALT), aspartate aminotransferase(AST), albumin (ALB), and total bilirubin (TBIL); Body character indexes: abdominal circumference, 24 hour urine volume, body weight, and time of ascites disappearance; Adverse reactions: any untoward medical occurrence during treatment.
