- This review researched the effectiveness and safety of Shugan jieyu capsule for the treatment of psychiatric symptoms in epilepsy.

- We will include randomized controlled trials assessing the effectiveness and the safety of Shugan jieyu capsule on psychiatric symptoms in epilepsy.
- Condition Being Studied
- Epilepsy is a chronic pathological condition of the brain that causes recurrent seizures. It is known that patients with epilepsy, particularly in the case of treatment-resistant epilepsy which accounts for one third to half, have an increased risk of psychological disorder. Psychiatric symptoms of epilepsy include not only depression, but also fatigue, sleep disturbances, anxiety, and memory problems. The relationship between epilepsy and psychiatric symptoms is known to be bidirectional. Clinically, epilepsy can influence the onset of depression through exposure to chronic stress, and depression increases the risk of epilepsy by 2 to 7 times. In addition, some antiepileptic drugs (AED) can either worsen or alleviate depression, which is still controversial. Anxiety and insomnia are also common symptoms of epilepsy, affecting epilepsy onset. AEDs can regulate the sleep-wake cycle by decreasing rapid eye movement and slow-wave sleep. Nevertheless, it is difficult for patients to aggressively receive adequate treatment for psychiatric symptoms. This is because there is no effective psychiatric drug criteria for patients with epilepsy, and there is a risk of exacerbating seizures. Moreover, psychiatric drugs are also revealed to worsen the side effects of AEDs. For example, even though depression in epilepsy is mainly treated with SSRI, FDA warns patients with seizures.
- Participant
- Participants who were diagnosed with epilepsy by the International Federation of Epilepsy in 1981 and complain of each psychiatric symptom (e.g. depression, anxiety, insomnia) verified by standardized criteria and/or scores of validated relevant questionnaires (e.g. Patient Health Questionnaire 9-item Depression and Hamilton Depression Rating Scale for depression) were included.

- Shugan jieyu capsule, a herbal medicine is composed of Acanthopanax senticosus and Hypericum perforatum. It has been widely taken to treat depression, cognitive disorder, stroke, functional dyspepsia, etc., since 2008 when it was registered. Acanthopanax senticosus reported to improve depressive disorder, ischemic stroke, and fatigue in previous clinical trials. Hypericum perforatum showed its effectiveness for depression clinically, and it is also reported to enhance cognitive function in preclinical study in rodents. The studies will be included if Shugan jieyu capsule was used solely or used with conventional treatment as an experimental intervention. In the case of combined form with conventional treatment, Shugan Jieyu capsule should be the only positive intervention and be compared with the same conventional treatment as a control group.
- Comparator Control
- All types of control such as placebo, conventional treatment or no treatment will be included.
- Main Outcome
- We defined the scope of psychiatric symptoms as depression, anxiety, and insomnia. (1) Depression will be measured with a validated depression scale (e.g. Hamilton Depression Rating Scale, Chinese-Neurological Disorders Depression Inventory for Epilepsy, Patient Health Questionnaire 9-item Depression). (2) Anxiety will be measured with a validated anxiety scale (e.g. Hamilton Anxiety Rating Scale, State-Trait Anxiety Inventory). (3) Insomnia will be measured with a validated anxiety scale (e.g. Pittsburgh Sleep Quality index, Insomnia Severity Index). (4) To find out antidepressant with the lowest risk of seizure exacerbation, frequency of monthly epileptic seizures will be measured. Measures of effect Mean differences
