- The clinical effectiveness of Chinese herbal injections in treating angina pectoris: protocol for a network meta-analysis of randomized controlled trials
- Review Question
- To evaluate the efficacy of available Chinese herbal injections (CHIs) for the treatment of angina pectoris.
- Study Type Included
- Inclusion: only randomized controlled trials (RCTs) will be included. Exclusion: quasi-RCTs, observational cohort and cross-sectional studies, post hoc analyses, case reports, experimental studies, reviews, duplicated studies, RCTs without information on the treatment regimens (e.g., follow-up periods) and RCTs without available outcome data.
- Condition Being Studied
- In China, cardiovascular diseases are estimated to account for 43% of deaths from non-communicable diseases, and of these, ischemic heart disease (IHD) is associated with among the highest mortalities of all the cardiovascular diseases. Angina pectoris is the most common symptom of IHD, and is characterized by transient episodes of chest pain or discomfort due to insufficient coronary artery blood supply. There are two main types of angina pectoris, unstable and stable, and anti-ischemia therapies including nitrates, beta-blockers and calcium channel blockers are recommended as conventional treatments.
- Participant
- Inclusion: adult participants are diagnosed as angina pectoris with the diagnostic criteria reported (e.g., World Health Organization (WHO) Definition of Ischemic Heart Disease, Diagnostic Criteria for Coronary Heart Disease released by Chinese Society of Cardiovascular Disease of Chinese Medical Association in 2007, etc.). Exclusion: participants without any symptoms of angina pectoris or those under the age of 18.

- Intervention
- Interventions: the one CHIs against angina pectoris is used in the intervention group. Exclusion: any RCT using multiple CHIs as the intervention or other Chinese traditional medicines.
- Comparator Control
- Comparators: another CHI or conventional treatments (such as beta-blockers, calcium channel blockers, and nitrates) is used in the control group regardless of combination therapy or monotherapy; the same conventional treatments can be used in the intervention group.
- Main Outcome
- Symptomatic influence (SYM-I) and electrocardiography influence (ECG-I). The outcome evaluation will be performed in accordance with Clinical Guideline of New Drugs for Traditional Chinese Medicine. Measures of effect SYM-I will be evaluated as the frequencies of attacking angina reducing and the daily nitroglycerin consumption both decreasing by 50%-80%. ECG-I will be defined as ST-segment elevation over 0.05 mV but normal range, the inverted T wave becoming less deep by over a quarter or the flat T wave changing to vertical.

- Additional Outcome
- Symptomatic recovery (SYM-R) and electrocardiography recovery (ECG-R). Measures of effect Symptomatic recovery (SYM-R) will be evaluated as symptoms disappeared or greatly relieved (e.g. the frequencies of attacking angina reducing and the daily nitroglycerin consumption decreasing by over 80%). Electrocardiography recovery (ECG-R) will be defined as electrocardiography after treatment being almost the same as the normal electrocardiography.
