Title
Efficacy and safety of sequential treatment with rhBNP-Qiliqiangxin capsule in heart failure: A Meta-analysis and quality evaluation of GRADE evidence
Review Question
Systematic evaluation of the efficacy and safety of recombinant human brain natriuretic peptide(rhBNP)-QLQX in the sequential treatment of heart failure.
Study Type Included
Only RCTs
Condition Being Studied
Heart failure(HF) is a serious manifestation and end stage of various cardiovascular diseases. It is a clinical syndrome caused by various kinds of cardiac damage that reduces cardiac output and cannot meet the metabolic needs of body tissues, with dyspnea, weakness and fluid retention as the main clinical manifestations, eventually leading to the decline of heart pumping function, with the characteristics of recurrent attacks and continuous progress.Patients with HF still have a high prevalence and mortality rate. Conventional drug treatment can improve symptoms in the short term but cannot reduce the incidence of rehospitalization, and side effects are inevitable.Qiliqiangxin capsule(QLQX) is widely used as a complementary and alternative treatment for HF. Systematic evaluation of the efficacy and safety of recombinant human brain natriuretic peptide(rhBNP)-QLQX in the sequential treatment of heart failure.
Participant
Subjects included in the study met the diagnostic criteria for heart failure in the Chinese Society of Cardiovascular Diseases.Patients whose interventions include Chinese herbal medicines, proprietary Chinese medicines or Chinese medicinal preparations other than Astragalus capsules will be excluded
Animal
Human Disease Modelled
Intervention
The experimental group and control group were treated with conventional therapy, including low-flow oxygenation, low-salt diet, correction of electrolyte disturbance, cardiac strengthening, diuresis, and anti-infection, antiplatelet, and lipid regulation, with the addition of rhBNP. The experimental group was treated with QLQX on top of the control group.
Comparator Control
The control group were treated with conventional therapy, including low-flow oxygenation, low-salt diet, correction of electrolyte disturbance, cardiac strengthening, diuresis, and anti-infection, antiplatelet, and lipid regulation, with the addition of rhBNP.
Main Outcome
The total clinical efficiency:according to the New York Heart Function Classification (NYHA) standard, after treatment, the patient's symptoms/signs improved significantly, heart function improved by grade 2 and heart function reached grade I as very effective, symptoms/signs relieved, heart function improved by grade 1 or heart function reached grade I-II as effective, no improvement of heart function or deterioration of heart function as ineffective; total clinical efficiency = (number of very effective + number of effective) / total number × 100%
Outcome Measure
Additional Outcome
BNP; NT-proBNP; cardiac function indexes (LVEF, LVESD, LVEDD); hemodynamic indexes (HR, SBP, DBP, CO, CI); myocardial injury markers (cTnI, CK-MB, hs-CRP); 6MWT; daily living ability score (BI);Safety indicators: incidence of adverse reactions.
