Title
Efficacy and safety of Qili Qiangxin capsule in treatment of pulmonary heart disease : a systematic review and meta-analysis
Review Question
To evaluate the efficacy and safety of Qili Qiangxin capsule in the treatment of pulmonary heart disease.
Study Type Included
RCTs of Qili Qiangxin capsule for patients with pulmonary heart disease were strictly included.
Condition Being Studied
Pulmonary heart disease (PHD) is a disease in which pulmonary vascular resistance is increased due to bronchial and lung tissue, thoracic or pulmonary vascular disease, resulting in pulmonary hypertension, followed by structural or/and functional changes in the right ventricle. PHD has a poor prognosis, which has become a rising major public problem that threatens people’s health and quality of life around the world. The etiology of PHD is complex and multifaceted and there is no single mechanism is responsible for PHD. At present, the predominant treatment for PHD includes low flow oxygen, infection control, bronchial dilatation, expectorant, small dose diuresis, cardiac strengthening, prevention of complications and other comprehensive methods. However, many problems are still common in patients, such as difficulty in effective control of infection, repeated fever or hemoptysis, progressive aggravation of pulmonary hypertension, and reduced immunity. At present, clinical studies and basic experiments have shown that Qili Qiangxin capsule can effectively improve the cardiopulmonary function of patients with cor pulmonale.
Participant
RCTs of Qili Qiangxin capsule for patients with pulmonary heart disease with NYHA grade greater than or equal toⅡ are strictly included. Gender, race, region and degree of illness are unrestricted. Exclusion criteria: Literature with incomplete or incorrect data; literature on combined treatment with other traditional Chinese Medicine other than Qili Qiangxin capsule; literature Unable to obtain full-text; the repeated published literature.
Animal
Human Disease Modelled
Intervention
The treatment group is treated with Qili Qiangxin capsule combined with conventional western medicine (produced by Shijiazhuang Yiling Pharmaceutical, orally, 4 capsules each time, three times a day).
Comparator Control
The control group received conventional treatment including low flow oxygen, infection control, bronchial dilatation, expectorant, small dose diuresis, cardiac strengthening, prevention of complications and other comprehensive methods .If the treatment group and control group have received routine treatment, the routine treatment must be consistent.
Main Outcome
Clinical efficiency, N-terminal pro-B-type natriuretic peptide (NT-pro-BNP), Pulmonary artery pressure including pulmonary artery systolic blood pressure (PASP) and mean pulmonary artery pressure (MPAP), left ventricular ejection fraction (LVEF), Right ventricular end-diastolic dimension(RVEDD), Right ventricular Tei-index, Adverse drug reactions. Measures of effect The relative risk (RR) should be used for the effect measure of clinical efficiency and adverse drug reactions. The mean difference (MD) should be used for the effect measure of NT-pro-BNP, PASP, MPAP, LVEF, RVEDD, and Right ventricular Tei-index.
Outcome Measure
Additional Outcome
Serum factor (hs-CRP and Copeptin), 6 minutes walking distance(6-MWT), the indicators of the arterial blood-gas analysis, lung function index(FEV1 /FVC). Measures of effect The mean difference (MD) should be used for the effect measure of additional outcomes.
