- Review Question
- To systematically assess the safety and effectiveness of Qili Qiangxin Capsule combined with sacubitril/valsartan in the treatment of HFrEF
- Study Type Included
- Randomized clinical trials will be included irrespective of blinding, publication status or language.
- Condition Being Studied
- Heart failure (HF) is the symptoms and/or signs of heart failure caused by abnormal heart structure and/or function, usually accompanied by elevated natriuretic peptide levels and/or objective evidence of cardiogenic pulmonary circulation or systemic congestion. And the objective evidences including imaging clinical syndromes such as physical examination, resting or load hemodynamic monitoring, etc. European Society of Cardiology (ESC) divided HF into 3 types namely HF with preserves ejection fraction (HFpEF, LVEF > 50%), HF with midrange ejection fraction (HFmrEF, LVEF 40–49%) and HF with reduced ejection fraction (HFrEF, LVEF < 40%) in 2016 . A report based on the global burden of disease showed that the number of patients with HF has increased from 33.5 million in 1990 to 64.3 million in 2017 , the increase almost doubled, and continues to grow rapidly due to the aging of the population. According to “China Cardiovascular Disease Report 2018”, there are about 5 million heart failure patients in China, and the 5-year average survival rate is about 50%, 10-year average survival rate is only 10%.Therefore, this study intend to systematically evaluate the efficacy and safety of Qili Qiangxin Capsule combined with sacubitril/valsartan for HFrEF, so as to provide reference for clinical practice.
- Participant
- Adults with HFrEF (as diagnosed by a clinician, or using any recognized diagnostic criteria) will be included.

- Intervention
- The intervention should be QQC combined with sacubitril/valsartan, whether it combined with other western medicine treatments or not.
- Comparator Control
- The control group should be sacubitril/valsartan, whether it combined with other western medicine treatments or not.
- Main Outcome
- Primary outcome measures including the left ventricular ejection fractions(LVEF), left ventricular end-diastolic dimension (LVEDD), left ventricular end-Systolic diameter (LVESD), N-terminal pro-B-type natriuretic peptide level(NT-proBNP), brain natriuretic peptide level(BNP), 6 minute walking distance (6-MWD), stroke volume (SV). Measures of effect Mean difference (MD) was used to represent continuous variable data; Odds Ratio (OR) was used to represent binary variable data. 95% confidence interval (CI) was used to evaluate each effect index. When the 95% CI of MD does not include 0 or the 95% CI of or does not include 1, the difference between the two intervention methods was statistically significant (P<0.05)

- Secondary outcome measure including aldosterone level(ALD), angiotensin II level (Ang-Ⅱ), and the adverse events. Measures of effect Mean difference (MD) was used to represent continuous variable data; Odds Ratio (OR) was used to represent binary variable data. 95% confidence interval (CI) was used to evaluate each effect index. When the 95% CI of MD does not include 0 or the 95% CI of or does not include 1, the difference between the two intervention methods was statistically significant (P<0.05).
