Star fruit and health: a systematic review of in vivo studies (Animal studies).
Review Question
Does the consumption of star fruit or fruit based products have a positive effect on health when assessed in animals? Context and rationale Chronic diseases such as diabetes and hypertension have been treated in ancient times with folkloric or traditional food. Averrhoa carambola, best known as star fruit due to its particular shape of five point star, is a tropical fruit used as an alternative medicine to control hypertension in some countries. For this reason there have been studies looking at its composition and bioactive potential, in addition to some clinical trials to understand the physiological mechanism. There have been reports on toxicity in individuals with renal disease after the consumption of star fruit. For this reason a review on the effects of star fruit consumption to identify the primary outcomes on animal models will provide mechanistic reasons for the possible health effects. A systematic review on this topic has not been conducted and it would provide a compilation of the literature findings to lead into further clinical research. This PROPRERO protocol will be under the same review for human studies (protocol ID = CRD42021272234).

Inclusion criteria: All in vivo experimental and controlled studies that involve star fruit or fruit based products on animals. Exclusion criteria: In vitro and ex vivo animal pre-clinical studies. Clinical trials, case series, case reports, cross-over studies, prospective and retrospective observational studies.

Inclusion criteria: All animal species (all sexes). Exclusion criteria: Ex vivo, in vitro and silico models. Human studies, as they are part of a separate registration (protocol ID = CRD42021272234).
Human Disease Modelled
Star fruit and its relationship with general health outcomes clinically measured.
Intervention
Inclusion criteria: Averrhoa carambola fruit or fruit based products such as juice or mixture of it with other natural products. Exclusion criteria: Averrhoa carambola leaves, roots, stems and non-fruit based products.
Comparator Control
Inclusion criteria: Baseline measurements, another intervention, vehicle-treated animals, non-exposed control group. Exclusion criteria: Sham-treated animals.

Outcome Measure
Inclusion criteria: Inclusion: General health outcomes clinically measured: body weight, blood parameters, urine parameters or faeces parameters. Tissue changes. Exclusion criteria: No relevant outcomes.
