- Review Question
- Is the herbal formula, Shen Ling Bai Zhu San, effective and safe for symptom managemnet of chronic diarrhea in adults?
- Study Type Included
- We will include randomised controlled trials (RCTs) that were reported in any language. Completed and ongoing trials will be included. Trials using a two-arm or three-arm parallel design will be included. Crossover trials and quasi-RCTs will be excluded. Any study with a sample size of less than 10 participants will also be excluded.
- Condition Being Studied
- Chronic diarrhea (CD) is defined as patient who has diarrhea (loose stools, increased stool frequency, or urgency) lasting longer than 4 weeks. These CD patients are mostly caused by non infections. It is still difficult in identifying some specific kinds of dirrahea and etiological treatment. Symptom manangment becomes physician's main goal. Conventional treatment includes opiate, octreotide, calcium polycarbophil, pectin, oral calcium supplementation, serotonin type 3 antagonist, chloride channel antagonist and other antidiarrheals. CD Patients are always suffering from unsatisfactory curative effects and side effects of routine treatments. Shen Ling Bai Zhu San (SLBZS), a traditional herbal formula, has been reported to be effective for mitigating CD symptoms in several clinical trails.
- Participant
- Inclusion: Participants who has been reported to meet the diagnostic criteria of CD. All eligible study participants will be included in this review regardless of their age, gender, race, sex, ethnicity, economic status or education background. There are no other limitations set for the exclusion of participants.

- Shen Ling Bai Zhu San (SLBZS) is a classical TCM formula which has been widely used in treating gastrointestinal disorders. SLBZS formula is composed of 10 herbs as follow: Panax Ginseng, Poria Cocos, Atractylodes Ovata, Diosorea Batasm, Coix lacbryma-jobi, Amomum Xantbiodes, and Platycodon Grandiflorum. Frequently administered to patients in clinical settings, however, the efficacy of SLBZS for diarrhea is unsubstantiated. In recent 5 years, new evidence is emerging with possibilities to change current evidence base. More than 10 clinical trials about SLBZS treating CD have been published. However, more rigorous appraisal is still lacking. Herbal formula based from SLBZS classic formula, allowing any dosage forms and modifications, will be considered in this study. SLBZS could be used alone or combined with CD pharmacotherapy. Studies where SLBZS are used combined with non pharmacotherapy therapy, such as acupuncture, massage, far infra red physical therapy, thermotherapy, magnetic therapy or pulse physical therapy, are excluded. Studies that compare the efficacy of different modifications of SLBZS or any herbal formulas will be excluded.

- Weekly bowel movements measured by validated instrument (Bristol Stool Form Scale, etc) Measures of effect Standard mean difference (or weighted mean difference) with 95% CI.

- 1. Stool consistency assesement through Bristol Stool Form Scale(BSFS); 2. Quaility of life; 3. Adverse events; Measures of effect For outcome reported in continuous data, SMD or WMD will be used. For outcomes with dichotomous data, relative risk or odds ratio will be used to measure the effect between group.
