Title
Does the use of vasopressin at time of excision of endometrioma improve ovarian reserve and reduce amount of blood loss intra-operatively?
Review Question
Does the use of vasopressin at time of excision of endometrioma improve ovarian reserve and reduce amount of blood loss intra-operatively?
Study Type Included
All randomized controlled trials comparing the use of vasopressin in ovarian surgery will be included. Observational studies (prospective or case-control studies) will also be included. Retrospective studies and case reports will be excluded. No language restrictions will be imposed.
Condition Being Studied
Ovarian reserve remains an area of concern when surgeries are performed on the ovary, particularly in the excision of endometriotic cyst as part of fertility treatments (in assisted reproductive techniques) to help conception. Surgical methods have been studied in the literature to minimise damage on ovarian reserve which include the use of bipolar device or suturing as well as the use of vasopressin and its studied effect on subsequent ovarian reserve. It is prudent that research is available to help guide clinicians with clinical practice.
Participant
Included population: Women aged 17 and over Use of vasopressin for ovarian surgery Measurement of operating time, haemostasis and effect on ovarian reserve Included study criteria: All randomised studies and prospective cohort studies looking at the effect of outcomes above on ovary. Surgery performed either laparoscopically or laparotomy Excluded population: • Malignant ovarian cysts
Animal
Human Disease Modelled
Intervention
The use of dilute vasopressin injection between the ovarian cyst and ovarian cortex at time of excision of endometrioma
Comparator Control
The use of normal saline injection between ovarian cyst and ovarian cortex at time of excision of endometrioma or cystectomy performed without the use of normal saline
Main Outcome
Ovarian reserve, using varying indicators, with ultrasound and biochemical markers such as ultrasound proven Antral follicle count, AMH level, FSH level Measures of effect Measures of effect is achieved through measurements of operation time, time taken to achieve haemostasis, effect of ovarian reserve from the 2 groups of normal saline vs vasopressin through measurement of biochemical markers of AMH/ FSH, or Ultrasound markers of antral follicle count.
Outcome Measure
Additional Outcome
Effect of vasopressin on operating time, effect on haemostasis, rate of complications Measures of effect measures of effect will be in minutes, mls of blood loss, or presence of any complications such as injury to visceral organs
