Review Question
What is the efficacy of Ocimum extracts in treatment of gingival and periodontal disease ?
Study Type Included
Inclusion criteria: • Randomized clinical trials, non randomized clinical trials. • Systematically healthy patients (>18 years), who have not been on antibiotic therapy, anti-inflammatory drugs, or have not undergone periodontal therapy in last 3 or 6 months. • Original publications investigating Ocimum extracts. • Articles which determined at least one of the following indices : gingival index, plaque index, pocket probing depth, modified gingival index, modified plaque index, sulcus bleeding index, clinical attachment level, relative attachment level as an outcome. • Articles reporting a minimum follow-up of 4 days. • Articles published in English language and between 1st January 2008 and 30th December 2020. Exclusion criteria: • In Vitro study. • Studies involving patients with adverse habits like smoking, tobacco chewing or alcohol consumption, pregnant or lactating women, patients undergoing orthodontic therapy or having removable prosthodontic appliances. • Only abstracts. • Review of literature, cohorts reviews, historical reviews, animal studies, editorials letters.
Condition Being Studied
Periodontal disease is inflammation of gums and its supporting tissue structure. The condition, is characterised by swollen and bleeding gums, with or without pain, mobility, bone loss and pocket formation. The most common cause of periodontal disease is dental plaque and calculus which is due to poor oral hygiene. Elimination of dental plaque, calculus and reducing the burden of bacteria prevents periodontal disease. Ocimum extracts have shown to have antibacterial, antifungal, anti-inflammatory, analgesic, antipyretic, antidiabetic, hepatoprotective, hypolipidemic, immunomodulatory, and cardioprotective activities. Numerous studies have been conducted on ocimum extracts in various forms like gel, mouthwash, dentifrices, and irrigating solution for treatment of periodontal diseases.

Systemically healthy adults above 18 years age having gingival or periodontal disease or both.

Ocimum extracts (Ocimum gratissium, Ocimum sanctum / Holy basil / Tulsi ) in the form of mouthwash, gel, irrigating solution.

Main Outcome
• To evaluate the efficacy of ocimum extracts in terms of plaque index (PI)/ modified plaque index , gingival index (GI)/ modified gingival index , sulcus bleeding index (SBI) in gingival disease. • To evaluate the efficacy of ocimum extracts in terms of pocket probing depth (PPD), clinical attachment level (CAL) / relative attachment level (RAL) in periodontal disease. Measures of effect Changes in plaque, gingival, sulcus bleeding index and pocket probing depth, clinical attachment level / relative attachment level scores from baseline to the last available follow-up, will be measured using means, standard deviations and percentage reductions
