- Review Question
- Does unrefined sugarcane affect inflammation in animal models with or without disease compared to healthy controls? Context and rationale Less refined foods, including sugars, contain more natural phytochemicals and fiber, that slowly digest and reduce glycemic response. Refined sucrose is the primary source of added sugar, widely used as a sweetening agent, preservative, viscosity-enhancing agent, and other responses in food and beverage. Nowadays, about 165 million tons of refined sugar is produced per year from sugar cane (80%) and sugar beets (20%). Scientists have developed non-nutritive and artificial sweeteners to reduce type 2 diabetes and obesity. Excess sugar consumption has been associated with weight gain, either by being preferentially stored as body fat or by accentuating appetite, which leads to overconsumption. Furthermore, the frequent high intake of added sugars combined with a sedentary lifestyle poses a high risk of developing metabolic syndromes, obesity, cardiovascular disease, diabetes, and dental diseases. Many studies reported that unrefined sugarcane products have antioxidant activity due to phytochemicals and polyphenol components. The antioxidants may decrease the risk of different inflammation and oxidative stress-related diseases such as cardiovascular, diabetes, obesity, several types of cancer as well as DNA damage. Based on the findings in animal studies, polyphenol can reduced the inflammation and oxidative stress mainly by KF-kappaB and Nrf2 pathways.

- Inclusion criteria: Case studies, controlled studies with a separate control groups, cross-over studies Exclusion criteria: Studies without a separate control groups

- Inclusion criteria: We will include all eligible studies that were conducted in animal models. Exclusion criteria: we will exclude in vivo studies and in silico studies.

- non-communicable diseases (NCDs) including diabetes, cancer, cardiovascular disease and obesity
- Intervention
- Inclusion criteria: We will include all animal studies that administered unrefined sugar in different form such as jaggery, brown sugar or molasses. Exclusion criteria: we will excluded studies that reported the effects of other sweeteners, dietary patterns, and glycemic index (GI). Furthermore, studies that simultaneously assessed the effects of other nutrients (such as herbal extract) with sugar intake will be excluded
- Comparator Control
- Inclusion criteria: We will include studies that assessed the effect of unrefined sugar compared to control group on inflammatory biomarkers. The following control group are eligible to include in our study: untreated, sham, treated with different intervention, vehicle treated and etc. Exclusion criteria: We will exclude studies without a control group.

- Outcome Measure
- Inclusion criteria: Outcomes variables will be inflammatory biomarkers including IL-6, TNf-alfa, IL-1β. we will report the findings of each study in the systematic review. Exclusion criteria: Those Outcomes that are not represent inflammation will be not reported.
