Effects of long-term cocoa consumption on cardiovascular health outcomes in older adults by pooling observational studies: a systematic review
Review Question
To investigate the prospective associations between long-term cocoa consumption and the risk and mortality of cardiovascular diseases and other related cardiovascular events in older adults aged 65+.
Study Type Included
• Inclusion criteria - Observational studies if they meet the following conditions: (1) the exposure was chocolate, cocoa or cocoa-derived products; (2) the exposure was at least a minimum of 3 months; (3) the outcome of interest was incidence of CVD and/or other related cardiovascular events (or associated hospitalisation/death); (4) the risk estimates of CVD and/or other related cardiovascular events, such as hazard ratio (HR), relative risk (RR), or odds ratio (OR) was reported; (5) the articles were published in English; (6) the articles were peer-reviewed; (7) the articles were available in full-text; AND/ OR (8) parts of paper did subgroup analyses by age. • Exclusion criteria - Qualitative studies - Cross-sectional studies - Studies that investigated the effect of other interventions along with cocoa-containing products but not in the control group - Studies without a control (non-exposure group) - Studies that were conducted on animals - Studies published based on books, letters, comments, conference papers, theses and reviews
Condition Being Studied
Cocoa/chocolate consumption in older adults aged 65+, Cardiovascular disease, coronary artery disease, heart failure, myocardial infarction, cardiac dysfunction, cerebrovascular accident including stroke and Transient Ischaemic Attack (TIA) and atherosclerosis.
Participant
• Inclusion criteria - Studies conducted on older adults with a mean age >65+, or all study participants aged >60. - Subjects at the onset of the study were free of any CVD or other cardiovascular events, which is the outcome of interests. • Exclusion criteria - Studies that were conducted on subjects aged <60, or with a mean age <65.

Intervention
In this study, the exposure of interest is cocoa or cocoa-derived products consumption.
Comparator Control
Cocoa or cocoa-derived products consumption will be compared to non-exposed group, which is the non-consumption group. High dose vs low dose analyses will also be included and analysed separately.
Main Outcome
The odds ratio, relative risk or hazard ratios of incidence of any CVD and/or other cardiovascular events (listed below), as well as CVD related mortality, in older adults with consumption of cocoa/cocoa-derived products compared to the non-exposure group (non-consumption group), as well as per each subtype. Outcomes related to subtypes of CVD in heart: • Coronary heart disease • Coronary artery disease • Heart failure (any type) • Myocardial infarction Outcomes related to subtypes of CVD in brain: • Stroke • Transient Ischaemic Attack Outcomes related to atherosclerosis: • Carotid artery stenosis • Aortic aneurysm • Peripheral Arterial Disease Measures of effect The measure of effect for the main outcomes will be odds ratio, relative risk or hazard ratios with 95% confident intervals (CIs).
