- Review Question
- Is zinc protoporphyrin testing accurate for determining iron deficiency in pregnant women and children?
- Study Type Included
- The present study will include all types of studies that have evaluated the relationship of ZnPP with iron deficiency in pregnant women and/or children. Studies that have reported ZnPP test (index test), performed concurrently with an eligible comparator test (enumerated above) will be included.
- Condition Being Studied
- Iron deficiency is common nutritional deficiency, responsible for more than 35 million disability adjusted life years (DALYs). It is a problem of public health significance in low- and middle-income countries, especially among vulnerable groups like pregnant women and children. Deficiency of iron in the body leads to incorporation of zinc (Zn) in the protoporphyrin IX molecule of haeme within the red blood cells which is amenable to fluorometric detection as zinc protoporphyrin (ZnPP). Measuring ZnPP levels is a minimally invasive, inexpensive and readily applicable approach for assessing iron status. Unlike other commonly used tests for identifying iron deficiency (viz. ferritin, soluble transferrin receptor, etc.), ZnPP measurement precludes the need of expensive instrumentation or trained manpower. ZnPP testing also has obvious advantages in comparison to gold standard methods like bone marrow iron stores (Prussian blue staining) which is invasive, time-consuming and not readily practicable. This makes ZnPP an attractive biomarker for determining iron status in resource-limited settings, including for point-of-care usage.

- Intervention
- The index test of interest for this review will be zinc protoporphyrin (ZnPP). ZnPP measured in capillary blood or venous blood, and expressed as microgram/gram haemoglobin or microgram/deciliter red cell or micromole/mole haemoglobin will be considered.
- Comparator Control
- The index test (ZnPP) will be compared against the gold standard method (i.e.., Prussian blue staining) as well as surrogate methods for testing iron status (viz., ferritin, total iron binding capacity, transferrin saturation, soluble transferrin receptor).
- Main Outcome
- Diagnostic accuracy of ZnPP in detecting iron deficiency in pregnant women and children. Measures of effect The measures of effect will include sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV).

- The expected additional outcomes are (i) accuracy of different thresholds of ZnPP for detecting iron deficiency in pregnant women and children, and (ii) association of ZnPP with other measures of iron deficiency.
