Title
Rhubarb-containing Chinese herbal compounds combined with trypsin inhibitor for severe acute pancreatitis: A systematic review and meta analysis
Review Question
To evaluate the efficacy and safety of rhubarb compound combined with protease inhibitor in the treatment of severe acute pancreatitis. To provide evidence-based reference for the treatment of severe acute pancreatitis.
Study Type Included
Clinical randomized controlled trial
Condition Being Studied
Acute pancreatitis is one of the common diseases in the department of gastroenterology. In recent years, with the improvement of living standards, morbidity and mortality are on the rise. The clinical symptoms mainly include abdominal pain, abdominal distention, nausea and vomiting, jaundice, fever, respiratory abnormalities, etc. At present, western medicine has certain advantages in nutrition support, anti-inflammatory, improving pancreatic microcirculation, inhibiting pancreatic secretion and so on. However, compared with TRADITIONAL Chinese medicine, it has great limitations in improving systemic symptoms and promoting gastrointestinal function recovery. Studies have found that rhubarb can protect the intestinal mucosal barrier and speed up the recovery of abdominal pain, abdominal distention, defecation and bowel sound in patients with acute pancreatitis. However, Traditional Chinese medicine compound is commonly used in the treatment of diseases with TRADITIONAL Chinese medicine. Therefore, this study will search existing domestic and foreign randomized controlled trials for systematic evaluation and Meta analysis of the trial results, to explore the clinical efficacy of rhubarb compound combined with protease inhibitors in the treatment of SAP, in order to provide a reference for clinical treatment.
Participant
Included in the standard (1) Study type: Clinical randomized controlled trial (RCT). (2) Subjects: All patients were in line with Consensus on Diagnosis and Treatment of Severe acute Pancreatitis, and there were no gender or age restrictions. (3) Intervention measures: The experimental group was given rhubarb Traditional Chinese medicine compound in addition to the conventional treatment in the control group, and the way of administration was oral, nasal feeding or gavage. (4) Control measures: protease inhibitors (octreotide, ulinastatin, somatostatin, etc.) and western medicine routine treatment, routine treatment includes fasting and water restriction, gastrointestinal decompression, anti-infection, spasmolysis and analgesia, maintenance of water and electrolyte balance and other measures. (5) Outcome indicators: The total effective rate, recovery time of abdominal distention, length of hospital stay and relief time of abdominal distention must contain one of the following indicators. Serum tumor necrosis factor -α, IL-6, C-reactive protein, and serum amylase must contain one 1.3 to rule out the (1) Repeated literature; (2) Non-Chinese and English literature; (3) Animal experiments; (4) Data cannot be extracted or full-text documents cannot be obtained; (5) accompanied by other organ failure; (6) The number of cases is less than 40.

Main Outcome
(1) total effective rate (2) abdominal pain relief time (3) abdominal distention relief time (4) hospital stay Measures of effect The counting data were presented as Odds Ratio (OR) and 95% confidence intervals (CI). Use MEAN difference (MD) as the effect analysis statistic for measurement data. Standardized mean difference (SMD) is used to describe the combined effect size of various studies if the units and measurement methods are different. The 95% confidence interval (CI) was calculated.
Outcome Measure
Additional Outcome
(1) Serum amylase (2) C-reactive protein (3) IL-6 (4) tumor necrosis factor -α Measures of effect The counting data were presented as risk ratio (RR) and 95% confidence intervals (CI). Use MEAN difference (MD) as the effect analysis statistic for measurement data. Standardized mean difference (SMD) is used to describe the combined effect size of various studies if the units and measurement methods are different. The 95% confidence interval (CI) was calculated.
