Review Question
Is it possible to make an early identification of situations with vital risk in patients who apparently are not serious?
Study Type Included
A prospective, longitudinal, observational study on patients, which are stabilized and evacuated in units of advanced life support to hospitals.
Condition Being Studied
Pathology of medical origin and traumatic
Participant
Patients with medical pathology or traumatic served by units of Advanced Life Support Inclusion: Will be included in the study all patients that are evacuated in units of Advanced Life Support to the reference hospital and do not meet any of the exclusion criteria. Exclusion:  •Patients Discharged in situ by the advanced life support unit •Patients attended by the advanced life support unit in the first instance, but evacuated by other vectors of transport. •Children under the age of 18 years.  •Cardiorespiratory arrest. •Pregnant Women in any week of gestation.  •Patients with a diagnosis of terminal stage in treatment for palliative care units.  •Psychiatric Patients or social problems.  •Time of arrival of the largest unit of 45 minutes.

Intervention
To all patients seen by a unit of advanced life support, will have the scale NEWS 2, and those patients that the medical team determine that must be evacuated to the hospital of reference, in addition will have a determination of a capillary lactic acid (mmol/L) using the Accutrend® Plus, Roche®, to determine the sum of the value of the scale NEWS 2 the quantitative value of lactic acid (mmol/L) to what we are going to be called NEWS2-L. Subsequently intends to carry out an analysis of the clinical history of the attention that have been generated for this intervention, valuing time, destination, additional tests, visits of specialists, early and late mortality, etc.
Comparator Control
In the present study there is no control group. only clinical data is collected and sorted in an effective way to give these data a clinical and statistical significance.
Main Outcome
To determine the usefulness of the scales of gravity NEWS2 and lactic acid for the value of the severity of the patients in the field of prehospital emergencies
Outcome Measure

•Know the demographic and clinical characteristics of the patients stabilized and evacuated by the units of advanced life support at the hospital level.  •Analyze whether there are significant differences in our study on the basis of the gender perspective.  •Determine the causes, type and time of the in-hospital mortality of this type of patients.  •Assess the diagnostic concordance in this group of patients among those carried out in the field of prehospital and hospital level. •Study the diagnostic and therapeutic measures that these patients require from the start of the prehospital care hospital until its resolution.  •Develop an analytical protocol prehospital: indications, management and interpretation.  •Implementation of a specific protocol prehospital sepsis is suspected, and continuity of care in the ER.
