- Review Question
- The primary objective is to compare the effects of pneumococcal conjugated vaccines (PCV) alone or added to a pneumococcal polysaccharide vaccine (PPSV) versus PPSV alone for the prevention of any pneumonia. Secondary objectives are:To compare the effect of PCV versus PPSV vaccine on any IPD and IPD caused by VT serotypes. To compare the effect of PCV versus PPSV vaccine on VT pneumonia and any pneumonia requiring hospitalisation. To examine the effects of PCV versus PPSV on immunological outcomes (antibodies level; opsonophagocytic activity). To compare the effect of PCV versus PPSV on serotype replacement. To compare the adverse events caused by PCV to the adverse events caused by PPSV.
- Study Type Included
- We will include RCTs and non-RCTs. The non-RCTs will be retrospective or prospective before-after studies and comparative clinical studies. We will allow adjustment through a matched cohort design, propensity score adjustment or adjustment through multivariable analyses.

- Participant
- Children and adults. We will include healthy participants and those with risk factors for IPD, including immunocompromised participants. We will exclude patients with HIV, who will be addressed in a separate review (Sadlier 2016).

- <ol> Any conjugated pneumococcal vaccine (7, 10, 13 or other valent). Any unconjugated, polysaccharide vaccine (23 or other valent if still in use).</ol> We will perform two comparisons:<ol> PCV alone versus PPSV alone. PCV + PPSV versus PPSV alone. We will consider separately studies in which PCV was followed by PPSV and those in which the order was reversed.</ol> Trials examining additional vaccines can be included provided that the additional vaccines are given equally to both study arms.
- Comparator Control
- We will perform two comparisons:<ol> PCV alone versus PPSV alone. PCV + PPSV versus PPSV alone. We will consider separately studies in which PCV was followed by PPSV and those in which the order was reversed.</ol>
- Main Outcome
- <ol> IPD or any radiographically confirmed pneumonia. Any radiographically confirmed pneumonia. </ol>
- Outcome Measure
- Additional Outcome
- <ol> IPD caused by any pneumococcal serotype and by VT. Pneumococcal pneumonia caused by any pneumococcal serotype and VT. Any pneumonia requiring hospitalisation. Immunogenicity at one month and one year post-vaccination (measured by antibody levels and opsonophagocytic activity). All-cause hospitalisations. All-cause mortality. Serotype replacement. Adverse events, including: <ol> local adverse events: tenderness, redness, swelling, limitation of movement, any local reaction; and systemic adverse events: fever (mild, moderate, severe), fatigue, headache, chills, rash, vomiting, new/aggravated generalised muscle pain, disease flare-ups, irritability, decreased appetite, increased or deceased sleep in infants. </ol> </ol>
