Review Question
1) How does physostigmine administration in patients presenting in an emergency medical setting with antimuscarinic toxicity affect the proportion requiring intubation? 2) How does physostigmine administration in patients presenting in an emergency medical setting with antimuscarinic toxicity affect the proportion requiring ICU admission?
Study Type Included
Randomised controlled trials, cohort studies including retrospective and prospective studies will be considered. Lower levels of evidence such as case series and singular case studies will be excluded.
Condition Being Studied
Antimuscarinic toxicity presents when a patient has an excess of an antimuscarinic substance introduced into their system. It may present with confusion, agitation, hallucinations, and carphologia, as well as dry mucous membranes, tachycardia, and mydriasis. This is commonly treated using supportive measures only, and may lead to higher than necessary rates of tracheal intubation and critical care.
Participant
Inclusion criteria: - Adults (patients aged 18 years or older) - Prehospital or emergency department setting - Antimuscarinic toxicity, or suspected antimuscarinic toxicity Exclusion criteria: - Peri-operative patients

Human Disease Modelled
Intervention
Any use of physostigmine, in an emergency setting, for known or suspected antimuscarinic toxicity.
Comparator Control
Standard supportive care. This extends to the use of sedatives, anti-epileptic drugs, and electrolyte solutions.
Main Outcome
1) Whether a patient was intubated or not 2) Whether a patient was admitted to ICU or not These dichotomous outcomes will be measured as per below. Measures of effect Results will be extracted as total numbers of patients, number of patients with the outcome, subsequent percentages, and odds ratios (where calculated).

Delirium control and use of CT imaging will also be investigated. Measures of effect Results will be extracted as total numbers of patients, number of patients with the outcome, subsequent percentages, and odds ratios (where calculated).
